What We're Looking For

Trillium Health Resources has a career opening for a Care Manager-RB-BHT to join our team! The Care Manager-RB-BHT is responsible for providing complex care coordination and monitoring to members having a primary Intellectual or Developmental Disability, which may include a secondary mental health or substance abuse, and/or complex medical/physical health need. The Care Manager-RB-BHT assists with integrated care management through multi-disciplinary collaborations and referrals, including but not limited to: assistance with the TCM opt in/opt out process, completion of care needs screenings, 90-day transition plans, completion/updates of CMCAs and care plan development, and service coordination as needed/deemed appropriate. This position has a primary focus on members that have been diagnosed with Autism Spectrum Disorder, particularly children (18mos - 15yo). The Care Manager-RB-BHT would be expected to work and be available at their remote location between the hours of 8:30 am - 5:00pm and be flexible to travel within the community to meet with members as well as attend other meetings as needed.

On a typical day, you might:

Serve as the primary point of contact for assigned members, providing ongoing care management and advocacy.

Conduct comprehensive assessments of member needs using person-centered planning methods and organizational tools.

Develop, implement, and monitor individualized care plans in collaboration with members, families, and multidisciplinary support teams.

Coordinate services across providers, agencies, and community resources to address member physical, behavioral, and social health needs.

Ensure assessments and care plans are updated to reflect changes in member needs and circumstances.
