This position exists to design, develop, and operationalize the new Continuity of Care legislation from 2026 session. The person will provide strategic vision and management for the comprehensive requirements that will require internal and external collaboration to ensure that Minnesotan's maintain their continuity of care. This includes people who need new services or care when impacted by a provider, agency, or similar transition, service disruption or termination, as well as individuals experiencing (or at risk of) institutionalization.
The position is embedded within the Transitions, Tribal & Transformations Division of the Aging and Disability Services Administration. While this position is state-funded, the division is primarily funded by the federal demonstration grant Money Follows the Person. This position will support both the demonstration and permanent functions and projects of the agency, administration, division, and demonstration grant.
Minimum Qualifications
Demonstrated leadership experience
Leadership experience can include:
Experience as a supervisor, leadworker, team lead, project lead;
Completion of a State of Minnesota leadership program (e.g. ELI, L4);
A paid/unpaid position leading a program or chairing a committee (e.g. ERG) - experience must include:
1+ year of experience identifying issues, goals and strategic actions
100+ hours per year time commitment
AND
4 years of professional work experience. Experience must demonstrate the following:
Extensive knowledge and/ or experience in delivery of the federal Medicaid program and Minnesota's Medicaid program, such as: Medical Assistance, home and community based services and programs (state and waiver plans, including, Home Care, Brain Injury-BI, Community Alternative Care-CAC, Community Access for Disability Inclusion-CADI, Developmental Disabilities-DD, Elderly Waiver-EW), the Americans with Disabilities Act, Olmstead Decision, Older Americans Act, Money Follows the Person grant, Housing, Mental Health, Substance Use, and other federal and state policy directions in long-term care, transition services, housing, employment, and community supports; and
Knowledge of state, county, Tribal and managed care delivery systems and funding streams
Management and strategic leadership processes, in particular those that support operations of a collaborative work relationships among state partner agencies and intra-agency divisions/administrations, and partner relationships; and
Experience in communications, human relations, and management to build collaboration and a network of committed partners and lead agencies
Preferred Qualifications
Organizational change management and continuous improvement, strategies, and applications
Medicaid provider operations related to program integrity, licensing, provider enrollment and similar systems
Negotiate and build consensus among the various internal and external partners on program policies and strategies in order to meet the state and project objectives and needs
Legislative processes, procedures and protocols that have broad, inter-related impact
Variety of experiences working effectively with others from different backgrounds and cultures
