Summary As a Program Analyst, providing support to the Primary Care Service Line operation at the Gulf Coast Veterans Healthcare System (GCVHCS). GCVHCS serve Veterans across three states, Mississippi, Alabama, and Florida. Our healthcare system consists of the Biloxi VA Medical Center, located in Biloxi, Mississippi: Joint Ambulatory Care Center, located in Pensacola, Florida; and three Community Based Outpatient Clinics located in Mobile, Alabama; Eglin, Florida; and, Panama City Beach, Florida. Responsibilities Serves as Primary Care Management Module(PCMM) Coordinator for Primary Care Teams within the Gulf Coast Veterans Healthcare System(GCVHCS) and its associated Community Based Outpatient Clinic (CBOC's). Responsible for ensuring the integrity and accuracy of the GCVHCS significantly large PCMM database which is used to organize and manage the GCVHCS Primary Care Network. Required to work with clinic support staff, clinic managers, Clinical Service Chiefs, Community Based Outpatient Clinic, CBOC staff, DSS/MCA Staff, VISN staff and Central Office staff to maintain the integrity of this program and data. Actively involved in local reviews leading to recommendations that many impact PCMM application processes. Provides input to VISN and national Program Managers on process challenges, program issues, and other problems so that these items can be addressed and corrected/improved. Primary responsibility for establishing and managing PCMM team and position set up for all Primary Care and Specialty PACT teams supervised by this Medical Center. Electronically create or modify team and position settings, assign provider user class based on verified credentials, set maximum panel sizes (based on analysis of provider licensure, FTE, patient intensity score, support staff, treatment rooms and specialty PACT adjustment factors), program clinical alert notification levels and establish preceptor to associate provider links and ensure proper reporting structure to national VSSC database. Uses multiple assignments/reassignment tools to move large groups of Primary Care patients to different teams and positions when changes become necessary. Monitoring and maintaining provider profiles, panel management including capacity, patient/ panel load leveling, suggestions with supporting data to the Primary Care Service Chief, oversight and correction of transmission efforts, and review/recertification of incorrect patient assignments, ensuring timeliness and accuracy of data input, monitoring tracking and management of any Primary Care Electronic Wait Lists (EWL) and identification and appropriate disposition of patient with duplicate PCMM assignments elsewhere within the VISN and VHA. Daily coordination with the Traveling Veteran Coordinator (TVC) to ensure services for Veterans outside their home catchment area are seamlessly provided needed care while on travel. Provides expertise and advice to all staff involved with PCMM, troubleshooting, and coordinating processes as needed. Trains, directs and assists Primary Care administrative staff in the appropriate assignment of patients to PACT teams and removal of patients from the database when discharged from Primary Care Collects, complies, analyzes and presents data regarding the integrity of the PCMM package. Identify strategic priorities for the PACT Programs and will track, monitor and analyze performance as needed. Will use performance data to identify areas for process improvement projects and works with Primary Care and clinic site leadership to identify any necessary resources for the successful implementation and management of such programs. Maintains communication with clinics site leadership at all CBOC's and sites of care to ensure timely responses and resolution to any PACT related issues. The incumbent plans and coordinates PACT improvement events. Assist in managing the NVCC PACT in PCMM. This entails adding information of the provider that the Veteran if referred to in the community. Will be back-up for the Primary Care Management and Program Analyst and the roles covered by that position to include Human Resource Actions, development of presentations and report, create responses for Action Items. Performs management analyses, research projects and provides advisory services on technical compliance, data integrity, workflow & process, and achievement of numerous and valued performance measure for Primary Care Service Line management and subordinate clinics. Work is accomplished on a weekly and/or monthly basis for compliance and data integrity, daily/weekly/monthly for numerous and varied performance measure activities, SAIL Star Teams and monthly or on ad-hoc basis for workflow and processes, with this work being quite frequent with the Primary Care Service Line emphasis on systems redesign and rapid cycle improvement across multiple Primary Care sites.
