Horizon Public Health serving Douglas, Pope, Stevens, Traverse and Grant counties is seeking applicants for a Full Time Accreditation Coordinator/Community Health Strategist. This position will coordinate and support upstream interventions to address the social determinants of health in our five communities. The strategist will analyze gaps, liabilities, opportunities and strategies in the @context of a specific population health challenge facing the community and contribute to grant-writing and work plan development aimed at eliminating community-based health disparities. In addition, this position will be responsible for coordinating and maintaining the national public health accreditation process along with planning, organizing, developing, administering, implementing, coordinating and evaluating the performance management and quality improvement system for Horizon Public Health. Minimum qualifications: Bachelor s degree in Public Health, Nursing, Health Education, Health Administration, Communications or a related field and moderate experience in public health or a related field. Successful applicant must pass a criminal background check and a pre-employment drug test. Must have valid MN driver s license and reliable, insured transportation. The starting salary range for this position is $38.19-$40.52/hour with earning potential up to $49.84/hour. General Definition of Work Performs intermediate work planning, organizing, developing, administering, implementing, coordinating, and evaluating a community health improvement strategy for Horizon Public Health. Responsibilities include identifying and analyzing data, connecting with internal and external partners and evaluating progress; complex organizational and administrative tasks related to the implementation of the Community Health Assessment, Community Health Improvement Plan, Performance Management, Quality Improvement Plan and the standards and measures required for accreditation, as well as all other work related to strategic planning, quality improvement, and accreditation as directed by the Public Health Administrator. The Community Health Strategist will lead the community health improvement efforts in partnership with health care and community leaders representing widely diverse sectors of the community. The Accreditation Coordinator shall coordinate the accreditation process within the health department and is the single communication contact between the health department and PHAB throughout the entire accreditation process. Work is performed under the general direction of the Public Health Administrator. Essential Functions Community Health Strategist: Coordinates and supports upstream interventions to address the social determinants of health in our communities. Participates in and supports external partners, community-based coalitions and internal departments to examine health data, set goals and develop plans to improve performance, population health and health equity. Engages and educates all staff and community partners to utilize health department plans and maintain the required Public Health Accreditation Board standards and measures. Leads the health department s accreditation efforts towards quality and performance improvement of services provided and a multi-sector, community-centered planning, integration, and implementation of strategies to improve the community s health and promote health equity throughout diverse populations. Coordinates a comprehensive community health assessment process by bringing together partners to develop, implement, and manage a Community Health Improvement Plan, which serves as a long-term, collaborative effort to address public health problems in our community; and collecting, analyzing and using data to educate and mobilize communities, develop priorities, garner resources, and plan actions to improve the public's health. Gathers comprehensive data (qualitative and quantitative) about the community's current health status, common needs, and issues by writing and updating the community health assessment as needed. Brings community partners together to develop, implement, and evaluate a community health improvement plan, which serves as a long-term, systematic effort to address public health problems in a community. Utilizes a systematic collection and analysis of data to provide a basis for decision making and to support the HPH communication team. Develops, implements, manages, and evaluates organization s strategic planning process. Develops the Performance Management and Quality Improvement Plan and responsible for implementing a comprehensive Performance Management and Quality Improvement process across all areas of the department, including internal committees and the Performance Management Council with the governing body. Coordinates teams and committees to measure and analyze program performance, customer satisfaction, the culture of quality processes, and accreditation standards and measures. Analyzes gaps, liabilities, opportunities, and strategies in the @context of a specific population health challenge facing the community. Contributes to the development of organizational competencies that strengthen the quality and performance of public health practice and participates in organizational capacity building and training activities to advance public health competencies. Supports the development, writing, and administration of the Strategic Plan with the Public Health Administrator, and carries out appropriate strategies and objectives. Contributes to grant-writing and work plan development to support community-based health disparities. Periodically presents to the governing board to inform on community health assessment, improvement, accreditation and performance management goals and progress. Attends all staff meetings, team meetings, Trainings, workshops, conferences, and staff development opportunities. Contributes to the development of organizational competencies that strengthen the quality and performance of public health practice and participates in organizational capacity building and training activities to advance public health competencies.
