Job SummaryUnder supervision of the Program Manager, the Admission and Transition Coordinator of the Restoration and Transition Unit will coordinate all aspects of the assessment and admission process for inpatient care for the Jail Based Restoration Program. In addition, the Admission and Transition Coordinator will lead and coordinate transition efforts to develop wrap around services for patients associated with competency restoration to ensure access to care and equitable outcomes. The Admission and Transition Coordinator will also oversee a substance use services in the Restoration and Transition Unit.The Correctional Care staff provides care at either Downtown Detention Center or the Denver County Jail. These environments of care have a higher level of clinical acuity and environmental hazard than inpatient and ambulatory settings based on the nature of the facility. The detention centers employ a high level of security, which impacts daily working conditions, including both personal and institutional safety and operations. Environmental awareness, critical thinking, and decisive, decision-making skills are required in the correctional care setting.Essential Functions:
Develop and implement a substance use curriculum to address those needs of patients in the unit. Provide groups and individual instruction regarding substance use and relapse prevention. Lead efforts to design and evaluate program performance regarding substance use programming. (40%)
Coordinate and track assessment and inpatient admissions to the Restoration and Transition Unit, including direct admits to jail-based restoration services. Duties include pre-admission screening for admission based on established eligibility for patient admission. Assist with the collection, documentation and transmission of admission and transition data and participate in performance improvement activities. Maintain communication with the Office of Civil and Forensic Mental Health, Forensic Services Team, and Jail Based Restoration teams in regards to admissions and transitions. Lead efforts to design and evaluate program performance regarding admission and transition using evidence-based methods. (25%)
Develop an individual program plan/contract with a patient including attending support/prevention groups and/or cognitive/behavioral classes, counseling, employment preparation, participating in community service activities, and/or referring patient to community agencies that meet particular individual needs and establishes goals with timelines for completing specific conditions. Determine on-going needs of the program and prioritize, plan, schedule and strategize new or modified goals and objectives (25%)
Develop and direct a robust community engagement plan that includes community professional partners in alignment with contract statement of work and available resources. Maintain and strengthen networking relationships. Develop and execute the program's vision and mission and ensure that all program objectives align with those of Denver Health, Denver Sheriff's Department, and the Office of Behavioral Health. (10%)
Education:
Master's Degree in Psychology, Sociology, Human Services, Criminal Justice, or a related field Required
Work Experience:
1-3 years of experience working directly with incarcerated clients/patients, preferably in a case management or health navigation role, Required
Licenses:
BLS-Basic Life Support (BLS/CPR) - AHA - American Heart Association or American Red Cross Required
Knowledge, Skills and Abilities:

Ability to identify problems, determines accuracy and relevance information, and uses sound judgment to generate and evaluate alternatives and to make recommendations. Knowledge of interviewing techniques sufficient to be able to elicit information.
Knowledge of the theories and practices of counseling and case management such as Stages of Change and Motivational Interviewing sufficient to be able to perform the duties related to the work assignment.
Knowledge of evidence of inclusion and principles of client confidentiality.
Skill in exercising initiative, judgment, creativity, and decision-making sufficient to be able to solve problems and meet client goals and organizational objectives.
Skill in calming and reassuring clients in order to engage them in a therapeutic relationship, the reentry process/services, and in motivating and empowering clients to self-monitor and self-manage.
Skill in demonstrating client advocacy practices and maintaining appropriate personal and professional boundaries with clients.
