Summary The Social Worker will serve as an integral member of the interdisciplinary Patient Aligned Care Teams (PACTs) at the Central Alabama Veterans Health Care System (CAVHCS). This position is responsible for providing comprehensive Social Work services to Veterans enrolled in PACT, with a specialized focus on those coping with substance abuse issues by planning and implementation of treatment processes tailored to the unique needs of Veterans with substance use disorders, Responsibilities Total Rewards of a Allied Health Professional Participates as a member of the interdisciplinary treatment team and actively participate through collaboration with Veterans and family as well as interdisciplinary treatment team members in the development and implementation of treatment goals and interventions. Facilitates action for community placements Serves as a liaison between Veterans and/or their families and VA and community resources in order to ensure thorough delivery of services. Educates Veterans and their families regarding community resources, VA benefits, and specialty programs, and advance directives. Maintains knowledge of Veteran's benefits and services, community resources, and process for making appropriate referrals to community and other governmental programs or agencies. Assesses and treat the complicated psychosocial problems of Veterans. Completes assessments to determine the presenting problem, to determine in-home factors impacting the problem, and to determine obstacles to treatment and/or follow through with recommended treatments. Works with Veterans and their families who are experiencing a wide range of complicated medical, psychiatric, emotional, behavioral, and psychosocial problems. Serves on committees, work groups, and task forces at the facility as deemed appropriate by the supervisor and Chief of Social Work Services. Provides assistance with the orientation and learning of less experienced Social Workers and, if interested, to serve as a field instructor for Social Work students. Participates in a rotation of on-call Social Workers, including modification of tour of duty to accommodate patient care needs on weekends. Keeps the Supervisory Social Worker and/or Chief, Social Work Service informed of program development, problems encountered, service needs, and positive or negative feedback. Provides age-specific care that is appropriate to the cognitive, emotional, cultural, and chronological maturation needs of the patient. Grade Determinations GS-11: Experience and Licensure: Appointment to the GS-11 grade level requires completion of a minimum of one year of post-MSW experience equivalent to the GS-9 grade level in the field of health care or other social work-related settings, (VA or non-VA experience) and licensure or certification in a state at the independent practice level. OR, Education: In addition to meeting basic requirements, a doctoral degree in social work from a school of social work may be substituted for the required one year of professional social work experience in a clinical setting.
