Physical Therapist - Home Health at Providence Home Services Oregon

Schedule: Full Time (40 hours/week), Weekdays on Day Shift (no weekends)

Area Coverage: Colombia River Gorge area

Manages and provides skilled physical therapy to patients/clients with a wide variety of diagnoses and disabilities in a home setting in accordance with a physician plan of treatment, taking into account individual patient special or age related needs. Requires skills in patient/client evaluation and treatment, as well as professional consultations to achieve maximum patient/client independence in accordance with the individuals functional level and potential.

Required Qualifications:

Bachelor's Degree from an accredited Physical Therapy program, or

Master's Degree from an accredited Physical Therapy program, or

D.P.T. from an accredited Physical Therapy program

Upon hire: Oregon Physical Therapist License

Washington Physical Therapist License - within 60 days of hire

Within 30 days of hire: National Provider BLS - American Heart Association

1 year clinical experience in Home Health or healthcare setting, or active enrollment in the Providence postgraduate preceptor program

Posted are the minimum and the maximum wage rates on the wage range for this position. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
