Summary The clinical pharmacy technician for medication reconciliation is a certified pharmacy technician with the ability to support the medication reconciliation program at the VA Northern California Health Care System (VANCHCS). Responsibilities The clinical pharmacy technician is responsible for performing medication reconciliation for patients under the direct oversight of the pharmacist assigned to medication reconciliation. The primary function of the medication reconciliation pharmacy technician is to assure and document the complete reconciliation of all medications. Major Duties include but are not limited to the following: Medication Reconciliation: Interviews patients (and families/caregivers as appropriate), to develop an accurate list of medications, strengths, and dosages the patient has been taking. Contacts other pharmacies to complete the accurate list. Assesses patient compliance with prescribed therapy through patient/caregiver interview and review of refill dates. Compares the compiled list with the active medication profile in VISTA/CPRS. Documents the compiled list in the electronic medical record, noting any discrepancies or omissions within 24 hours of the patient encounter. Notifies the overseeing pharmacist of any discrepancies, omissions, questions, or concerns regarding the patient. Other Clinical and Pharmacoeconomics: Prioritizes operations and assigned clinical functions to provide the highest quality of patient care. Reports all potential patient or pharmacy distribution problems verbally and/or in writing as requested by the pharmacist in the area assigned. Reviews medication profiles for prescription duplication, therapeutic duplication, prescription noncompliance, and allergies / adverse drug reactions. Reports identified patients to assigned pharmacists for further evaluation. Provides education for patients concerning operational issues or need for therapy compliance. Documents clinical and operational activities as assigned f. Participates in quality improvement processes and programs as assigned. Grade Determinations: In addition to the basic requirements above, applicants must meet the following grade requirements. Experience Requirement. Candidates must possess one year of experience equivalent to the next lower grade level of GS-7. Creditable Experience. To be creditable, a candidate must possess the required knowledge, skills and abilities (KSAs) associated with the scope of pharmacy technician practice. The candidate's experience must be evidenced by active professional practice. (Active professional practice means paid/nonpaid employment as a professional pharmacy technician.) Part-Time Experience. Part-time experience is creditable according to its relationship to the full-time work week. For example, one week of full-time credit is equivalent to two weeks of half-time work. Quality of Experience. Qualifying experience must be at a level comparable to pharmacy technician experience at the next lower grade level. For all assignments above the full performance level, the higher-level duties must consist of significant scope, administrative independence, complexity and range of variety as described in this standard at the specified grade level and be performed by the incumbent at least 25% of the time.
