Summary The Community Care Service (CCS) coordinates skilled care to Veterans purchased from outside community vendors. The CCS works collaboratively with local VA Medial Center providers, providers at other VA Medical Centers, and with local community vendors including home care, hospice agencies, local hospitals, long term care facilities, etc. The purpose of the work is to complete specific procedures and processes, which effect accuracy, reliability, and productivity of the Community Care Service. Responsibilities This position relieves the supervisor and clinical staff of various routine and administrative work processes and ensures work complies with appropriate policy and procedure. Duties include but are not limited to: Responsible for telephone and written correspondence with providers, outside vendors, and local community hospitals and agencies. Maintain program supplies and equipment, including independently identifying needs and making acquisitions. Places appropriate orders and tracks requests through delivery. Responsible for receiving, tracking and appropriately forwarding or processing incoming outside medical records, treatment plans, outside provider orders. Establish a monitoring system for tracking, admission, discharges, authorizations and extensions, non-VA services purchased through the Community Health Program. Responsible for collecting, compiling and tracking statistical information on to support the program operations, including productivity/workload reports, patient/ purchased care tracking reports Additional associated duties as assigned. Experience coordinating medical documentation, referrals, authorizations, or patient scheduling in clinical or administrative settings. Familiarity with electronic health records (EHR) systems, patient intake processes, or medical office workflows. Demonstrated ability to manage high volume administrative tasks such as documentation processing, tracking, data entry and communication with patients or providers. Strong customer service and communication skills, with experience interacting with patients, families, healthcare staff, or external providers. Ability to prioritize multiple tasks, maintain accuracy, and support fast paced healthcare options. Physical Requirements: The work is primary sedentary; typically, the incumbent is comfortably seated to perform required work. The position requires some physical effort including standing, walking, bending, filing, lifting, or carrying light weight items such as files, mail, and supplies in and through all parts of the facility. Required to answer numerous phones and personal inquiries and must have adequate eyesight, manual dexterity to allow for reading, operating computer, fax, printer, and other similar office equipment.
