Duties and Responsibilities (these duties are a general summary and not all inclusive): Deliver comprehensive nursing care to adult patients with complex and chronic medical conditions, including those with acute care needs. Conduct thorough wound assessments and develop evidence-based treatment recommendations in accordance with clinical guidelines. Lead the implementation and ongoing refinement of wound care and skin integrity protocols aligned with national standards. Collaborate with interdisciplinary teams to develop, evaluate, and adjust patient-centered care plans focused on wound management and fall prevention. Chair the Skin Wound Assessment Team (S.W.A.T.) and oversee routine wound prevalence studies to monitor outcomes. Direct and manage the facility's Fall Prevention Program, including leading committee meetings and disseminating performance data. Analyze clinical data related to wounds and falls, preparing reports to support quality improvement and benchmarking initiatives. Investigate and evaluate incidents involving patient falls and skin breakdown, identifying trends and recommending corrective actions. Provide education, training, and guidance to nursing staff on best practices in wound care, fall prevention, and performance improvement. Evaluate emerging products and clinical practices, conduct trials, and present recommendations to support evidence-based care and regulatory compliance.
