Schedule: Full Time, Monday-Thursday 4:00PM-12:00AM, Every Sunday 8:00AM-4:00PM

Job Description

Assists all phases of the referral, intake and admissions processes to ensure timely and appropriate admissions of rehabilitation outpatients. Coordinates and monitors the department master therapy schedule.

Job Responsibility

Facilitates patient's entry by monitoring and tracking required information and admissions criteria, including physician prescriptions, managed care documents, and financial clearance.

Maintains outpatient rehabilitation master schedule and 'waiting list'.

Monitors occupancy rate and fills all schedule vacancies, as needed.

Assists with Patient Financial Services personnel to ensure timely and appropriate financial clearance for prospective new admissions.

Tracks and monitors patient attendance.

Coordinates and directs externally referred patients based on patients needs.

Provides written and verbal information to prospective patients.

Collects and organizes referral and admission data.

Troubleshoots intake admissions process to resolve issues and resolve process.

Job Qualification

High School Diploma or equivalent required.

1-3 years of relevant experience, required.

The salary range and/or hourly rate listed is a good faith determination of potential base compensation that may be offered to a successful applicant for this position at the time of this job advertisement and may be modified in the future. When determining a team member's base salary and/or rate, several factors may be considered as applicable (e.g., location, specialty, service line, years of relevant experience, education, credentials, negotiated contracts, budget and internal equity).

The salary range for this position is $29.91-$33.45/hour
