The Informed Choice and Support Planning Team Lead will support development and coordination of case management, informed choice, support planning, and public guardianship policies on behalf of the Disability Services Division (DSD) Support Planning and Informed Choice Team. The incumbent works with the case management, informed choice, support planning and public guardianship subject matter experts (SMEs) on crafting legislative proposals, policy development and implementation goals. Responsibilities include:

Lead strategic planning, coordinate workplan efforts

Provide recommendations that are aligned with the DSD vision and goals and equity initiatives

Provide regular reporting to leadership regarding DSD, statewide and national efforts & best practices

Minimum Qualifications

3 years of specialized professional experience providing or administering case management services. Experience must include the following:

Experience with Minnesota's Medical Assistance and long-term care programs and their related systems, financing, providers and federal and state laws, regulations and requirements

Knowledge of Minnesota case management systems and processes, as they relate to the long-term care service system

Experience with project management, process improvement, program redesign and related approaches

Preferred Qualifications

Demonstrate leadership sufficient to establish work goals and expectations, anticipate problems, plan contingencies, evaluate results, provide feedback, and advise others

Advance skills in Microsoft Office Suite (Word, Excel, Access, PowerPoint, Visio, SharePoint, Project Management Software), ability to learn, understand and apply new technologies

Plan, manage and facilitate meetings; understand divers, sometimes competing, concerns of stakeholders, interest groups and constituents; develop and utilize facilitation strategies to foster collaboration

Coordinate highly complex initiatives, activities, and policies. Lead planning, analysis and program administration efforts and groups

Variety of experiences working effectively with others from different backgrounds and/or cultures
