Summary The VA Health System, has 1 vacancy for a full-time Registered Nurse (PACT Outpatient Clinic, Croasdaile CBOC). The RN will be fully qualified and certified to function in a clinical role and in accordance with the guidelines and policies of the Veterans Affairs Health Care System. Responsibilities The Durham PACT Outpatient Clinic (Croasdaile) maintains the health and manages the care for patients with a variety of medical problems including hypertension, diabetes, arthritis, cancer, congestive heart failure and other chronic diseases. The clinic is designed to provide comprehensive primary/specialty care services, including gender specific care to male and female veterans. The clinic is staffed by providers (attending physicians, mid-level providers), registered nurses, licensed practical nurses, pharmacists, social workers and medical support assistants. The Registered Nurse will serve as a primary care nurse in support of the physician or mid-level provider. Duties will include, but not be limited to, oversight of nursing staff, patient intake, documentation, point of care testing and health screening. The RN will assess, triage, take histories and document conditions and concerns for veterans presenting to the clinic and via the telephone. The RN is responsible for promoting an environment that supports caring and courteous service, safe patient care, and positive working relationships. The clinical nurse will coordinate care through the health continuum using health education, health coaching, and health promotion. Preferred Qualifications Minimum of three years of successful clinical practice. Prior recent experience in ambulatory care/primary care nursing, including care of patients with mental health and health education needs. Previous experience in the care manager role and coordinating care/care delivery. Previous experience with telephone advice nursing. Demonstrated proficiency in using the nursing process in providing care for clients with complex care needs. Previous knowledge/experience with the patient centered medical home practice model. Demonstrated ability in the areas of interpersonal relationships, problem solving/critical thinking skills and conflict resolution. Qualifications Basic Requirements: English Language Proficiency. Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE); OR Individuals attending a master's level bridge program in nursing who have completed coursework equivalent to a bachelor's level degree in Nursing may have opportunity to become registered as a nurse with a state licensing board prior to completion of the bridge program. Upon achievement of a State license, the individual may be appointed on temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program. Credit for foreign nursing education higher that associate degree/entry level requires a formal degree equivalency validation from a recognized equivalency evaluation accepted by VA such as International Consultants of Delaware (ICD). Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Graduate Nurse Technician (GNT) Exception: Candidates who otherwise meet the basic education requirements, but do not possess the required licensure, may be appointed at the entry step of the grade and level applicable to the completed nursing education as a GNT on a 120-day temporary appointment while actively pursuing licensure (may be extended up to two years on a case-by-case-basis.)
