CO State Veterans Community Living Center at Rifle
The Colorado State Veterans Community Living Center at Rifle is located on Colorado's western slope in the scenic Colorado River valley. Rifle offers a mild climate, with plenty of clean mountain air and beautiful blue skies. Amidst this setting, the State Veterans Community Living Center at Rifle provides 89 beds of skilled nursing care, including a 18-bed Special Care Unit for residents with Alzheimer's Disease or related conditions. Serving veterans, their spouse and eligible "Gold Star" parents, we are committed to caring for the whole person by addressing the personal and social needs of each resident, as well as their physical and medical requirements. All staff members of Rifle are committed to providing a life of qualify and dignity for every resident. At the Colorado State Veterans Community Living Center at Rifle, our staff is known for showing love and compassion to our residents. Description of Job
Shift Information: Wednesday-Saturday, 7:00 am-5:30 pm
Program Facilitation
Receives direction from supervisor and then plans and facilitates recreational programs that are designed to met the interests and needs of each individual resident and/or neighborhood. Prepares the recreation calendar for the neighborhood and presents it to the supervisor for approval. Programs will be varied to meet a variety of residents individual interests, which may include cognitive, recreation, physical groups, spiritual experience, creative endeavors. Prepares and facilitates daily groups for the neighborhood assigned.
One to One Programs
Provide 1 to 1 therapeutic programs to residents with decreased ability to interact with their environment as care planned by the Clinical Therapist, which may include hand massage, sensory stimulation, wheelchair walks, visits about the facility, utilization of the sensory cart, artistic expression, etc.
Program Coordination
Coordinates activity program times, content, and location with nursing services, housekeeping, and dietary staff. Organizes kitchen requests for special functions with the dietary department and linen request through laundry services. Acquires supplies needed for programs. Maintains purchasing card within state and facility guidelines and within budget for the department. Conducts outings of choice by the residents and ensures safety of the residents while outside the facility.
Documentation
Timely completion of all documentation including MDS interview, full assessments, event notes, 1:1 visits, activity attendance, and monthly calendars. Maintain informational binders/spreadsheets such as: Resident Histories, Outing List, Participations Logs and Pet Inoculations, and a variety of spreadsheets. Complete signs -post and distribute for care partners viewing.
Supply Management
Responsible for Activity Department supplies related to purchase, organization, cleaning, and repair. Follow P-Card Guidelines and document expenses. Provide all receipts for activity purchases and turn in promptly to the Accounting Department. Assist resident to purchase personal items. Follow Resident Benefit Fund guidelines for resident activities and supplies.

Minimum Qualifications
Education: Must possess a current, valid CNA license or valid practical nursing license (LPN) from the Colorado Department of Regulatory Agencies or any state participating in the Nurse Licensure Compact
Preferred Qualifications:
Previous Long-term care/nursing home experience is highly desirable
Ability to effectively work with an elderly patient population of varying physical, mental and emotional abilities and staff from multiple work units in the facility to achieve quality care planning and delivery
Ability to follow patient confidentiality standards
Excellent interpersonal and communication skills
Ability to read and comprehend the English language
Ability to effectively communicate fluently in English both verbally and in writing
Ability to follow care plans
Conditions of Employment:
FULL BACKGROUND CHECK : This position will have direct contact with vulnerable persons (as defined by C.R.S.27-90-111: "Direct contact" means providing face-to-face care, training, supervision, counseling, consultation, or medication assistance to vulnerable persons, regardless of the level of supervision of the department employee. "Direct contact" may include positions in which persons have access to or unsupervised time with clients or patients, including but not limited to maintenance personnel, housekeeping staff, kitchen staff, and security personnel.)
Full background checks include :
CBI & FBI fingerprint check, Colorado Court Database check, Medicare Fraud Database, TRAILS Database check.
This position will also provide direct care to vulnerable adults requiring a CAPS check (as defined by C.R.S. 26-3.1-101, position provides services & support care to at-risk/vulnerable adults including case management, clergy, physical or mental health services, safety or welfare services)
This position requires State of Colorado residency at the time of application (unless otherwise identified in the posting), and residency within the state throughout the duration of employment in this position.
Motor Vehicle Check
Must possess a valid, non-restricted Colorado Driver's License or a non-restricted U.S. Driver's License for positions within 30 miles of the CO state border
Out-of-State Driver's License holders must obtain a valid, non-restricted Colorado Driver's License within 30 days of employment start date.
Must complete and pass TB test and any other vaccinations as required by the facility.
