Summary:
The Program Manager Patient Safety reports to the PMPH Director Patient Safety, Quality Regulatory and Risk and works in partnership with both the director and the PMPH Patient Safety Officer to coordinate, monitor, and continuously improve the execution of PMPH's Patient Safety Program. This role manages the event reporting system for PMPH, reviews data for trends that might require action, oversees serious event management processes and outcomes including root cause analyses (RCAs) and will work with Risk Management to ensure thorough and efficient event evaluation and institutional response.
This role supports important safety committees and subcommittees both locally and at the Penn Medicine System level. Works with the Director of Quality, Patient Safety, Regulatory and Risk locally to support, implement and follow up on action planning relating to the patient safety culture survey. This role works across clinical and non-clinical departments across PMPH to enhance the patient safety environment. Along with the PMPH Director Patient Safety, Quality, Regulatory and Risk, the Patient Safety Manager will represent PMPH at the UPHS Event Management Council
Responsibilities:
Serves as entity manager of Penn Medicine's event reporting system, by routing event reports and onboarding of PMPH staff and providers, as needed.
Reviews reports for trends that may require action, including reports that monitor manager and director event management and follow up. Serves as PMPH "watchdog", ensuring appropriate assignment of harm score and event management routing.
Coordinates and as needed, facilitates serious event management processes and their outcomes, including root cause analyses (RCAs), in conjunction with the PMPH Director of Patient Safety, Quality, Regulatory and Risk and PMPH Patient Safety Officer to ensure thorough and efficient event evaluation and institutional response, including necessary internal and external reporting (New Jersey Department of Health and voluntary reporting such as The Joint Commission).
Supports the Patient Safety Committee in its oversight of the PMPH patient safety plan, including, but not limited to, coordinating the monitoring of compliance to standards supporting the National Patient Safety Goals, Sentinel Events, Leapfrog Hospital Survey, Culture of Safety, FMEA and patient safety related projects.
In conjunction with Health Information Management and the Performance Improvement team ensure accurate coding and reporting of Patient Safety Indicators and identify occurrences where clinical or performance improvement follow- up and action are warranted.
Works with Director of Quality, Patient Safety, Regulatory and Risk to coordinate all local efforts that support the implementation of the Patient Safety Culture Survey and develop, maintain, and refine standardized reports to support and improve Patient Safety culture and measurable outcomes, and present these reports and other information, as needed, to relevant departments and/or committees
Serves as an entity manager of Origami, Penn Medicine's potential compensable event database. Gathers necessary information and acts as a liaison with the liability insurance carrier regarding all potential and verified claims.
Supports Director of Quality, Patient Safety, Regulatory and Risk Coordinates in facilitating the release of records in response to subpoenas, insurance, or attorney requests.
Supports Director in working with appointed hospital legal counsel regarding the investigation of forthcoming proceedings, and acts as a liaison with hospital employees concerning their interviews/depositions, when appropriate, to prepare defense claims against the hospital.
Advises the PMPH Director of Patient Safety, Quality, Regulatory and Risk and PMPH Patient Safety Officer of any complex issues or problems involving patient safety and recommends corrective actions.

Salary Range:
$86,306.69 - $137,585.76/Annually based on 40 hours per week
