PUEBLO REGIONAL CENTER (PRC)
This work unit exists to ensure the Agency complies with all state and federal regulations relating to Food and Nutrition Services; and to provide complete Food and Nutrition services to the residents living in Pueblo Regional Center group homes. Services include but are not limited to: Medical Nutrition Therapy (assessing nutritional status, determining the nutrition diagnosis, determining appropriate nutrition interventions, and monitoring and evaluation outcomes), nutrition counseling, meal planning, and guidance for safe, nutritionally adequate, cost-effective and quality meal service. The unit is also responsible for instructing direct support staff on food safety and sanitation, food buying principles, inventory control, and therapeutic diets. The unit dietitian is responsible for monitoring food service and food budget expenditures for the homes.

About the Position
This position exists to meet Waiver regulations by addressing short-term and long-term nutritional needs of intellectually disabled individuals through nutritional assessment, development, and implementation of nutritional care plans; to evaluate the effectiveness of the plan with the interdisciplinary team and to work with other team members in transitioning clients to the community. As required by Medicare/Medicaid standards, this will be accomplished by the Nutrition Care Process and Medical Nutrition Therapy.
Primary Job Duties
Medical Nutrition Therapy.
Screens, assesses, and monitors each ID individual's nutritional status for optimal health including, but not limited to, individuals with chronic medical conditions, inborn errors of metabolism, oral-motor dysfunction, and individuals with a wide range of moods and psychiatric behaviors. Provides physicians and other healthcare professionals with a formal nutrition assessment as part of the direct patient care for dietitians outlined in the Centers for Medicare & Medicaid Services Guidelines. This position has the primary responsibility for tracking and reporting the resident's weights, assessing the reasoning/cause for significant changes, and working with appropriate individuals to implement changes as needed.
Nutrition Counseling.
Counsels individuals and direct care staff that care for the individual on nutritional issues such as diabetes management through diet. Conduct training and in-service education to direct care staff on medical therapeutic diets, food safety and sanitation, food buying principles, inventory control, food preparation, and texture modification for an individual's diet.
Menu Individualization.
Develops and maintains individualized menus for clients to meet the requirements for medical nutrition therapy as well as individual preferences. Adapts recipes to meet the nutritional and oral-motor needs of individuals within the allocated budget.
Dietary Audits and Nutrition Charting.
Conducts regular audits of group homes to monitor all dietary practices that directly affect an individual's nutritional status, food safety, sanitation and meal service. Additionally, charts on residents to reflect the latest Medical Nutrition Therapy implementation.

Experience Only:
Five (5) years of relevant experience in an occupation related to the work assigned to this position

Education and Experience:
A combination of related education and/or relevant experience in an occupation related to the work assigned to this position equal to five (5) years
Special Qualifications:
A Master's degree in an accredited dietetics program.
A registered Dietitian license Through the American Dietetic Association is required for this position.
Preferred Qualifications:
Experience as a mentor, trainer, or supervisor over healthcare staff.
Experience applying person-centered practices.
Experience writing assessments and working with Individual Service and Support Plans (ISSP).
Working knowledge of HIPAA rules and regulations.
Computer proficiency.
Writing assessments and following Individual service and support plans.
Team Player
Excellent Customer Service Skills

Shift Work - PRC is a 24/7 facility, must be available to work any shift, any day and/or any area, to meet the a agency needs.

Motor Vehicle Check , if the employee will drive a state-owned vehicle, either in an on-going capacity while performing their normal day-to-day job duties OR on occasion for training/meetings/client visits/etc.

Travel - Position must travel between remote work sites and may be required to transport individuals. Must utilize a State vehicle, if available. If not, must utilize a personal vehicle and request mileage reimbursement. A state vehicle must be utilized when transporting individuals.
Special Qualifications - A Master's degree in an accredited dietetics program. A registered Dietitian license through the American Dietetic Association is required for this position.
Training - Must demonstrate the ability to lift 50 lbs., bend, kneel, squat, etc. and perform interventions proficiently throughout employment.

Employment history is calculated on a full-time basis (40/hrs per week). Part-time employment is calculated on a prorated basis to determine qualifications.
