Summary The Social Worker will be responsible for providing services to veterans presenting for care at the St. Louis VA Medical Center. This includes high quality case management, clinical treatment, advocacy, and coordination of linkages to other appropriate VA and/or community service providers / agencies as needed by the client. The incumbent demonstrates skill in the provision of services to address veterans who tend to have serious and severe crises, may lack any familial and community support, may be poor self-monitors, may frequently fail to comply with instructions and treatment, or may have major deficits in coping skills and require continuing professional psychological support. Work involves intensive social work services requiring the exercise of mature professional judgment and the flexible use of a wide range of social work practice skills; individual, group, family therapies. The incumbent will have gained a range of specialized interventions and acts as resource expert to colleagues. Duties include, but not limited to: Complete an assessment of the veteran in collaboration with the interdisciplinary treatment team, the veteran, family members and significant others. Interview patients, develop a psychosocial assessment of the veteran in collaboration with the interdisciplinary treatment team, the veteran, family members and significant others. Responsible for developing the treatment/care plan setting achievable treatment goals with the veteran/family and prescribing provider. Responsible for educating the veteran and/or caregiver of the available services and assisting them in establishing the appropriate referrals based on the veteran's preference or that of his surrogate decision-maker. Responsible for using the current social work resources of the VA and community social service programs and enhancing the content to the benefit of veterans. Understands the intimidation of bureaucracy and will act as an advocate when it serves the best interest of the veteran/family. When appropriate and feasible, incumbent will educate and encourage the veteran/family to advocate on their own behalf, thus fostering a sense of independence and empowerment and belief in personal growth. making rapid assessments and developing crisis management plans for admission to medical/surgical/geriatric programs for complex issues. Provides education related to VA and community resources, entitlements, Advance Directives/Living Will and will refer veterans/families to the appropriate interdisciplinary team member for identified health education needs. This requires judgment and skill in utilizing supportive, problem solving, or crisis intervention techniques. Ability to establish and maintain effective working relationships and communicate with clients, staff, and representatives of community agencies. This includes acute, chronic, and traumatic illnesses/injuries; common medications and their effects/side effects; and medical terminology. Social Worker, GS-11 (Full Performance Level) Experience and Licensure: Appointment to the GS-11 grade level requires completion of a minimum of one year of post-MSW experience equivalent to the GS-9 grade level in the field of health care or other social work-related settings, (VA or non-VA experience) and licensure or certification in a state at the independent practice level.
