Responsibilities The Patient Aligned Care Team (PACT) LPN is an essential member of the PACT Team. Duties include but are not limited to, daily and 2-week scrubbing, patient check-in, completion of clinical reminders, and 2-day post Emergency Department follow-up calls. LPN's provide coverage in the Lab and specialty clinics during absences. The LPN provides a warm hand off to the provider following check-in.
