Job Summary
As Supervisor, provides day-to-day operations support and supervision to the Care Transition Specialists; leading and coordinating referral through-puts while challenging the team to achieve excellence. Responsible for consultant service delivery.As Consultant, obtains and prompts conversion of referrals to admissions and facilitates a smooth transition of patients into Continuing Care facilities and Homecare services. Maintains communication with referral sources, external case managers and insurance companies, and assists with the coordination of services.Provides input to the Senior Director regarding budget, referral/admission barriers and strategies, staffing and customer service. Establishes, monitors and evaluates the performance goals of the liaisons. Encourages individual and team growth among the staff while providing high quality, compassionate service. Must be capable of traveling to and from referral sites located off campus.

Provides input to the Director or Sr Director regarding budget, referral/admission barriers and strategies, staffing and customer service. Deployment of business development, sales, communication and marketing tactics.
Establishes, monitors and evaluates the performance goals of the Care Transition Specialists. Encourages individual and team growth among the staff while providing high quality, compassionate service. Recruits, evaluates and hires staff as necessary.
Leads referral process and staff work process development/continuous improvement.
Qualifications
Required
Associate's Degree or equivalent in nursing, therapy, or social work
2 years of relevant skills, knowledge, abilities typically gained through related experience
LIC-Registered Nurse (RN) - STATE_MI State of Michigan required Or
LIC-Master Social Worker (MSW-Master) - STATE_MI State of Michigan required Or
LIC-Physical Therapist - STATE_MI State of Michigan required Or
LIC-Physical Therapist Assistant - STATE_MI State of Michigan required Or
CRT-Occupational Therapist, Registered (OTR) - NB-COT National Board for Certification in Occupational Therapy required Or
CRT-Occupational Therapy Assistant, Certified (COTA) - NB-COT National Board for Certification in Occupational Therapy required Or
CRT-Speech Language Pathologist - ASHA American Speech-Language-Hearing Association required Or
LIC-Bachelor Social Worker (BSW) - STATE_MI State of Michigan required Or
LIC-Bachelor Social Worker Limited - STATE_MI #N/A required Or
LIC-License Practical Nursing (LPN) - STATE_MI State of Michigan required
Preferred
Bachelor's Degree or equivalent in nursing, therapy, or social work
2 years of relevant experience working with insurance companies and government payers
2 years of relevant progressive prior supervisory experience and prior experience with persons requiring home care and/or hospice services
3 years of relevant experience as a case manager
