Job Coach - Facility Job Description

Job Coach works to promote community integration, skill acquisition, and independence by providing individualized on the job training and support to maintain employment.

Essential Functions:

Provide on- the job training, and on-going support by implementing and supporting employment related goals and training strategies from Individualized Support Agreements and related support plans

Providing information based on observation and interactions with client and feedback from supervisor to the individual, employment consultant, and others in support team for evaluation of client progress and support needs.

Re-strategizing with the client to implement most effective daily work plan.

Providing transportation to work sites

Advocating with client for new learning experiences on the job.

Working collaboratively and consistently as a team member to promote a positive, professional and respectful team approach and working environment.

Provide environmental or job modifications that will promote client safety and productivity on the job.

Assist with development of a phase out plan

Attend regularly scheduled meetings and supervision

Complete all needed documentation

Other duties as assigned

Driving required

Requirements:

High School Diploma or equivalent

At least one year of experience in human services or combination of education and/or experience from which comparable knowledge and skill has been acquired

Valid driver's license and use of a personal vehicle

Competencies:

Communication - Triages communication needs as they arise. Accurately communicates and reports in all formats.

Intercultural Sensitivity - Takes an active interest in others, their cultural background, needs and perspectives.

Organization/Attention to Detail - Manages, organizes, and prioritizes essential functions. Follows agency policy and procedures. Identifies what needs to be done, and takes initiative.

Service to Others - Understands, supports and meets the needs of clients with compassion and empathy. Navigates system of care across populations. Maintains confidentiality of client information.

Technology - Develop and maintain proficiency in the use of office equipment, hardware and software. Demonstrates knowledge of agency information management and security policies.

Physical Requirements:

Medium Strength rating 20-50lb occasionally, 10-25lb frequently, and 10-20lb constantly

Ability to Frequently kneel, crouch, squat, twist, and bend throughout the day

Ability to sit and stand frequently (34%-66% of workday) throughout the day

Ability to walk frequently throughout the day

Frequent (34%-66% of workday) use of Manual dexterity for tasks such as ability to make quick coordinated movements of one or both hands to grasp and manipulate objects.
