OBJECTIVE: The primary purpose of the TMA is to administer prescribed medications and limited topical treatments to residents under direct supervision of a registered nurse or licensed practical nurse. Perform routine resident care duties delegated by the registered nurse or licensed practical nurse, to meet the physical and psychosocial needs of residents. Maintains all required documentation for medication and treatment administration. TMA reports any noted changes in the resident as they are noted. These services are in compliance with all state and federal regulations and policies and procedures by the facility. QUALIFICATIONS. A minimum of high school diploma or equivalent is required. Registered or certified as a nursing assistant. Completion of a state approved training program in medication administration with certificate received. Effective communicator with staff, families and outside agencies. Must possess the ability to make independent decisions when circumstances warrant such action and as delegated by the licensed nurse. Ability to prioritize duties. Customer service oriented with knowledge of successful customer service techniques. Ability to understand resident's charts, doctor's orders, resident's plan of care, medication orders and to communicate with residents and staff. Must possess the ability to effectively gather clinical data in regards to resident status and condition and report to licensed nurse. Numerical ability necessary for proper measuring of medication and to monitor specific resident cares. Must be able to safely perform the essential functions of the job without harm to self or others. Must be eighteen (18) years of age or older. Able and willing to work flexible hours, such as during an emergency situation. Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing and documentation. ESSENTIAL FUNCTIONS: Administration of medications and treatments as delegated by a licensed nurse. a. Follows all shift routines, policy and procedures regarding measuring of vital signs, weights and administration of medications and treatments. Follows facility medication protocols. Obtain report, getting full information on residents' conditions and new orders. Confer with team members concerning assignments, routines, and rehabilitation of residents. Discuss with charge nurse any necessary changes. Prepare, administer and chart medications accurately, timely and safely according to policy and procedures. Count narcotics accurately. Performs non-invasive treatments as directed (i.e.: ointments, Ted Hose application, splint application, PROM exercises per facility policy and procedures). Utilizes pharmacy, drug references and licensed nurse for medication administration questions. Maintains an awareness of actual and potential side effects, adverse reactions, contra-indications, indications and purposes of medication. Communicate with licensed nurse throughout shift concerning resident's medication regime, refusals and special observations. Alerts nurse prior to administering a PRN medication, licensed nurse must first be consulted. The licensed nurse will be responsible for recording results of the PRN. Documents response to the PRN medication administered. Takes vital signs, as indicated, reporting any abnormalities to nurse. Perform treatments for which he/she has been properly trained and which has been assessed and directed by charge nurse on a daily basis. Notify the charge nurse of any noted change in status. Maintains a clean medication/treatment cart, clean and ready for use at all times. Medications replaced appropriately. Follows strict medical asepsis protocols. ADL assistance to residents as assigned and observation of the resident's general status. Reports any changes in the resident's functional ability to the nurse. Reports any indication of illness or change in status to the nurse. Recognizes and protects the confidential nature of resident information. Provides any information on changes in skin conditions to the nurse. Assist and observe in the Dining Room at mealtime. Observe and report any noted changes in mental status to the nurse. Assist with mealtime and reports changes in appetite, chewing or swallowing difficulty. Monitors completion of NAR or HHA required documentation (i.e., walk list, food and fluid intakes records, bowel status). Report to charge nurse all incidents, accidents that occur involving residents, visitors and staff. Reports to charge nurse observations of residents' conditions requiring further assessment. Monitors unit, accessibility of call lights and other general cleaning duties of NAR's or HHA's. l. Answers call lights promptly. Treats and ensures that residents are treated with respect and dignity. Immediately reports any suspected abuse or neglect per facility policy. Refers resident and family questions and concerns to the charge nurse as necessary. Assist with admission, transfer and discharge responsibilities not related to assessment. Documentation: Behavior charting per facility policy. Maintain twenty-four (24) hour report on assigned unit. Accurate documentation of medications and treatments administered and performed according to all facility policy and procedures. Assist to maintain information lists (i.e., weights, vital signs, BM, B+B, nourishment, intake and output, etc.). Personnel Functions: Attend designated nursing meetings and in-services, as scheduled. Keep abreast on new medications, indications and potential side-effects. Develop and maintain a good working relationship with inter-departmental personnel. Promote harmonious relationships and favorable attitudes among the health care team. Attends required in-services and completes assigned online modules. Maintains medication and treatment supplies in designated areas. Reports need for additional supplies to supervisor.
