Bereavement Program Specialist

Essential Responsibilities \' Coordinate bereavement services, develop
and update care plans for patients\' families up to one year after
death. \' Provide counseling, education, and crisis support for
patients, families, and staff; facilitate support groups and memorial
services. \' Assess spiritual needs, develop individualized
spiritual/pastoral care plans, and collaborate with local clergy. \'
Serve as an active member of the interdisciplinary team (IDT) and
participate in care planning and conferences. \' Provide referrals and
support for complicated grief, and offer grief education to staff,
patients, families, and the community. \' Conduct or arrange
funeral/memorial services and maintain a network of clergy resources. \'
Maintain accurate documentation and comply with HIPAA, Medicare, and
professional standards. \' Practice within scope of licensure, maintain
professional boundaries, and adapt to various care environments. \'
Participate in staff orientation, mentoring, and offer guidance on
spiritual/pastoral issues. \' Serve as an ambassador for CareOregon and
Housecall Providers. Organizational Responsibilities \' Perform work in

Adhere to the organization\'s policies, procedures and other relevant
compliance needs. \' Perform other duties as needed. Experience and/or
Education Required \' Masters degree in theology, divinity, religion,
pastoral care, philosophy or health related field and minimum 2 units
clinical pastoral education (CPE) \' Minimum 2 years hospice experience
as a hospice or palliative care spiritual counselor/chaplain,
bereavement coordinator, grief counselor, hospital chaplain or similar
setting \' CPR certification at hire or within 6 months in position \'
Valid driver\'s license, acceptable driving record, and automobile
liability coverage or access to an insured vehicle Preferred \' Minimum
4 or more units of clinical pastoral education (CPE) \' Board
certification or board certification in process through the APC, NACC,
NAJC, or SCA \' Experience with electronic medical records Knowledge,
