ACT Care Coordinator (QMHP)

Job Summary:Integral Care has four ACT teams that support clients with complex mental and physical health needs, who frequently use emergency health services and/or interact with the criminal justice system. Integral Care's ACT team includes social workers and counselors who focus on mental health and case management, a Housing Specialist, a Substance Use Counselor, a Peer Support Specialist, a Registered Nurse and an Advanced Practice Nurse.

The team serves adults who live with mental illness and use drugs or alcohol, focusing their care on three specific areas. Individuals who experience:

Frequent and intensive psychiatric emergencies

Chronic homelessness

Frequent involvement with the criminal justice system

The ACT teams fit their services to the needs of the client. The complexity of those needs require that the team is involved in all aspects of client care - routine healthcare visits, crisis episodes, incarcerations and psychosocial needs including housing, employment and family support.

The team promotes health and well-being by providing access to treatment, rehabilitation and support services. Services include 24-hour mental health crisis support, one-on-one emotional support, medications, housing support, access to medication, nursing services, drug and alcohol treatment and help building daily living skills. The team brings services to clients at home or in the community.

Integral Care's ACT team works with primary healthcare providers, hospitals, Austin Police Department, defense attorneys, payees/guardians and more to support the health and well-being of clients.

On-call responsibilities may be required dependent upon unit protocols. This position may have additional duties associated with providing bilingual communication services and requires the completion of a language competency test.

Regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule.

Education:

Required:

Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field.

License:

Required:

Current and valid driver's license and a clean driving record.

Experience:

Health/Human Services experience preferred.

Bilingual preferred.

Hours:Full-Time.

Workdays:Monday - Friday (on call rotation weekends/holidays).

Workhours:8:00AM - 5:00PM (on call rotation after hours).

Salary:$24.18- $25.39 an hour based on education plus an additional 10% crisis stipend.
