This is a full time, benefits eligible, hourly position, working 80 hours bi-weekly.

Primary Purpose

Develop each resident's program with the assistance of the Inter-Disciplinary Team, in accordance with each resident's specific needs and supervise these programs.

Essential Job Functions

Participate in the pre-admission evaluation and in the admission procedures for each resident.

Develop an Individual Habilitation Plan (IHP) for residents and will complete 90-day notes on each resident scheduled monthly and include monthly tabulation of goal data in Point Click Care. Evaluate the resident's programs based on the documentation of all staff involved with the resident, will be responsible for documentation of the IDT meeting for assigned residents, and will develop and complete the preparation of the Annual Program Plans for caseload assigned.

Assist in the supervision of individual programs for residents assigned, serve as team leader at IDT meetings for residents assigned, and coordinate all services relevant to assigned residents.

Participate as team leader with the IDT, particularly in areas of scheduling, program development, problem solving, and functional assessments for residents assigned.

Integrate, coordinate and monitor active treatment programs for each resident, including after school programs, recreational, social and leisure activities, and give assistance to Residential Service supervisors and Direct Care staff as needed to ensure execution of these programs.

Participate as point person for resident related incident reports; investigate and implement appropriate follow-up training.

Other Job Functions

Coordinate the scheduling, planning and collating of information for staffing on each resident assigned. Will ensure staff are trained on resident program changes for each annual plan, or at any time a change is made to the plan.

Ensure that the resident receives those services and interventions necessary by competent persons capable of delivering them.

Review and revise as needed individual program plans and transmit this information to members of the IDT.

Attend all IEP's for residents to establish a carry-over of services.

Facilitate and take minutes for assessments, IHP meetings, and other team meeting requests.

Collect and systematically record and analyze data to justify needed changes in a resident's program plan.

Participate in the internal audit of residential and programming records.

Facilitate communication between the Administration, therapies, and support services.

Participate in the In-Service Training programs at the request of the Administrator.

Represent the facility at meetings of various Commonwealth, Archdiocesan, Regional, and County agencies, when requested by the Administrator.

Assist licensing surveyors with information and records for caseload assigned.

Will be responsible in securing the IDT's input on the correction of deficiencies encountered by licensing surveyors and projected dates of correction.

Education

Bachelor's Degree required

2-years Intellectual Disability experience (preferred)

1-2yrs QIDP experience (preferred)
