At the State of Minnesota, employees play a critical role in developing policies, providing essential services, and working to improve the well-being and quality of life for all Minnesotans. The State of Minnesota is committed to equity and inclusion, and invests in employees by providing benefits, support resources, and training and development opportunities.
The Sage Screening Programs within the Health Promotion and Chronic Disease (HPCD) division seek a Clinic Network Specialist to lead statewide planning and coordination that strengthens community clinical linkages across more than 440 clinic provider sites. This position provides expertise in operational planning, systems design, and implementation of evidence-based strategies to improve access to breast, cervical, and colorectal cancer and heart health screening in Minnesota. This role serves as the primary clinic network lead for both Sage, MN's National Breast and Cervical Cancer Early Detection Program (NBCCEDP) and SagePlus, MN's WISEWOMAN Program.
The position drives health equity and improved screening outcomes by guiding clinics through workflow redesign, designing and delivering comprehensive network training, supporting systems change initiatives, and leading partner clinics' transition to the new Sage Link data system across multiple submission tracks. The Clinic Network Specialist bridges Sage and SagePlus clinic and community partners, conducting environmental scans, gathering clinic feedback through office hours, webinars, visits, and assessments, and providing technical guidance to program leadership. This position works collaboratively with clinics, EBI-support partners, and community organizations to strengthen screening systems and improve cancer and cardiovascular prevention services in priority communities across Minnesota.
Primary responsibilities include:
Serve as the principal planning expert, systems authority, and network training lead for the Sage Programs clinic screening network of over 440 sites, directing long-range strategic planning and operational goal setting, developing health systems frameworks, implementing network-wide training initiatives, and driving direct patient screening services, while collaborating with the section's Cancer Screening Quality Improvement lead to align data and workflows.
Serve as the clinic network lead for the implementation and coordination of clinical-community linkage strategies and cardiovascular screening workflows for the SagePlus program, collaborating closely with the SagePlus Director, Program Coordinator, and Lifestyle Intervention Coordinator to align site-level activities with overarching programmatic goals.
Coordinate clinic-level health systems frameworks to support patient navigation linkages, partnering with clinic personnel to establish and optimize electronic health record (EHR) workflows that enable patient navigators to systematically identify unscreened patients, conduct proactive outreach, and evaluate individuals for Sage and SagePlus program eligibility.
Design, execute, and evaluate innovative approaches for community-clinical linkage projects across the network, exercising independent planning authority to align multi-organizational protocols and maximize population health outcomes.
Lead the training for the statewide 440+ Sage provider network to guide clinics through their transition to the new Sage Portal system which includes using varied data submission methods. Collaborate with the Sage Data Manager and MNIT to establish long-range strategic frameworks for future direct Electronic Health Record (EHR) data transfers.
In accordance with HR/LR Policy #1422 Telework, this position is eligible for up to 50% telework for applicants who live in Minnesota or in a bordering state within 50 miles of the assigned work location.
