THE POSITION

DESCRIPTION OF WORK
This position oversees key care management activities that support older adults throughout the community. It ensures that services are well-coordinated and delivered in accordance with program requirements. As an Aging Care Management Supervisor 1, you will perform the following duties:
Staff Supervision:Supervise Aging Care Managers who provide care management to older adults
Eligibility Review:Determine client eligibility for programs and make appropriate referrals
Report Preparation:Prepare monthly, quarterly, and annual reports on department status
Service Monitoring:Monitor services under contract such as home support, personal care, and legal services
Agency Coordination:Maintain contact with community agencies and exchange information on services for the elderly
Work Schedule and Additional Information:
Full-time employment
Work hours are 8:00 AM to 4:00 PM, Monday - Friday, with 60-minute lunch.
Telework: You will not have the option to telework in this position.
Salary: In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY
QUALIFICATIONS
Minimum Experience and Training Requirements:
Three years of experience in public or private social work and a Bachelor's Degree;or
Any equivalent combination of experience and training including successful completion of 12 semester hours of college level courses in sociology, social welfare, psychology, gerontology, or other related social sciences.

This position falls under the provisions of the Older Adult Protective Services Act.
Under the Act, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance.
