Empowering People's Independence (EPI), provides support for people with

developmental disabilities, epilepsy, and brain injury through progressive, tailored care to improve their health, wellbeing, independence, and social inclusion.

Job Purpose:

The primary purpose of this position is to schedule and coordinate various medical, dental, and counseling appointments for persons supported at EPI.

Hiring Range : $20.00/hr-$25.50/hr

Essential Functions:

Maintain the health and well-being of the persons supported at EPI.

Schedule and coordinate all medical appointments for the persons supported at EPI, including acute appointments - transport and accompany people to all appointments, assist with video appointments.

Notify circle of supports of all appointments and results of appointments.

Ensure medical provider documents the results of appointments and recommendations on all appropriate forms and send copies of consults to appropriate personnel.

Alert Resident Nurse (RN) and supervisor of any updates needed to Individualized Plan of Protective Oversight (IPOP) or Plan of Nursing Services (PONS).

Assist in obtaining and maintaining needed adaptive equipment/supplies.

Complete weekly medication inventory.

Complete monthly medication check for expired medications.

Update files such as provider list, personal information sheet and consult forms.

File purge medical paperwork.

Provide medications to Day Programs.

Report all policy violations to appropriate personnel.

Promote workplace safety.

Performs other duties as assigned by the Program Supervisor and/or designee within scope of practice.

Non-Essential Functions:

Participate on various EPI committees as able.

Compliance:

Must maintain confidentiality and exercise discretion concerning all business-related matters, including but not limited to board/vendors/donors' names, investigations, finance, operations, and employee relations matters.

Must be willing to complete specific company required trainings as mandated by our governing agency, Office for People with Developmental Disabilities (OPWDD), at time of hire and annually thereafter.

Must adhere to all departmental /company policies & procedures at all times.

Education and Experience:

High school graduate or equivalent and one-year experience in the human service field

Supervision:

None

Computer Skills:

Strong working knowledge of Microsoft programs to include Email, Word, Excel, PowerPoint, Teams.

Qualifications:

Must possess and maintain a valid NYS Driver's License-a vehicle is required for business travel

Solid verbal and written communication skills and ability to communicate with all persons supported and a diverse employee population of various levels within the organization.

Demonstrated ability to use poise, tact, maturity and diplomacy, during difficult and/or stressful situations to effectively collaborate and resolve issues.

Proven ability to work in a fast-paced environment, with demonstrated ability to manage multiple tasks, and customer requests, while meeting deadlines.

Must become Van certified.

Ability to assist in wheelchair transfers.

Must be able to talk, hear, stand, walk, and/or sit for extended periods of time; may have to twist, bend, use hand to finger, stretch with hands and arms.

Specific vision abilities required by this job include close vision and ability to adjust focus

May be exposed to strong odors from time to time.

Must be able to lift and carry up to 50 lbs.

While performing the duties of this position, exposure to changing weather conditions will occur.
