Ability to recognize, assess, deescalate and respond to behavioral expressions such as agitation, wandering, resistance to care, verbal aggression, or other responsive behaviors. Demonstrates the ability to complete nursing assessments including the Veterans risk factors; physical, psychosocial, emotional and mental status; medications; environmental and safety and educational needs Develop initial plan of care and review the interdisciplinary plan of care with all team members during the quarterly team meeting. This plan of care is reviewed every 90 days during the IDT meeting, or as needed based on changes in the patient's condition. Collaborates with pharmacist through interdisciplinary team meetings, or as needed based on quarterly pharmacy review of medications. Monitors the use of oxygen therapy and provides education to patient regarding safe use of oxygen. Completes treatments as ordered, which may include catheter changes, dressing changes, and administration of injections as ordered. Manages the care of patients, while organizing and prioritizing patient care needs, and ensuring assigned tasks are completed. Actively contributes to the collaboration process and clearly communicates ideas to promote team success. Makes meaningful and challenging contributions for the betterment of the work group.
