Summary VA Greater Los Angeles Healthcare System seeks an experienced RN for a dual case management and clinical nursing role in Imaging. The RN provides case management and care coordination for Veterans undergoing Interventional Radiology, Diagnostic Radiology, and Nuclear Medicine procedures and treatments, with case management as the primary responsibility and current emphasis on IR. Duties include scheduling, education, barrier resolution, IV access, medications, monitoring, and sedation support. This position serves a dual case management and clinical nursing role within the Imaging Service, with case management and care coordination as the primary responsibilities. The RN will manage care for Veterans undergoing Interventional Radiology, Diagnostic Radiology, and Nuclear Medicine procedures and treatments, with current emphasis on Interventional Radiology. Responsibilities include coordinating care from referral and scheduling through preparation, procedure completion, and follow-up; assessing patient readiness; reviewing orders and clinical documentation; resolving barriers to care; triaging urgent requests; providing patient education; and communicating with Veterans, caregivers, and clinical teams. As a secondary component of the role, the RN will provide direct clinical nursing support, including pre-procedure assessments, IV access, medication and contrast administration, patient monitoring, sedation-related care within established policies and competencies, and recovery and discharge support. Job duties including, but not limited to: Coordinate and assist with scheduling imaging and procedure appointments. Ensure appropriate patient preparation (instructions, NPO guidelines, medication holds, labs, consent verification, etc.). Review orders and supporting documentation for completeness. Triage urgent and emergent procedure requests from the Emergency Department, inpatient units, and clinics. Communicate preparation instructions and expectations to patients and clinical teams. Reviews renal function labs (BUN/Cr/eGFR) for contrast exams and determines when updated labs are required. Screens for anticoagulant/antiplatelet use and verifies appropriate medication holds per protocol, especially for lnterventional Radiology procedures. Reviews allergy and contrast reaction history and ensures premedication protocols are verified and followed when indicated. Assist with patient monitoring during imaging-related procedures, including procedures involving moderate sedation (within policy and competency). Support safe workflow and communication between technologists, providers, and procedural staff. Reinforce adherence to safety protocols, timeout procedures, and contrast safety guidelines. Performs vital-sign monitoring for MRI patients with pacemakers/lCDs when required. Assists with anxiolytic/minimal sedation for MRI/CT (e.g., lorazepam), including pre-, intra-, and post-monitoring per facility policy. Provide immediate post-procedure follow-up for inpatients and outpatients. Assess patients for post-procedure concerns or complications and escalate as appropriate. Coordinate necessary follow-up appointments, repeat imaging, or additional diagnostic/interventional studies. Ensure appropriate documentation, communication, and patient instructions after procedures. Qualifications Basic Requirements: English Language Proficiency. Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE); OR Individuals attending a master's level bridge program in nursing who have completed coursework equivalent to a bachelor's level degree in Nursing may have opportunity to become registered as a nurse with a state licensing board prior to completion of the bridge program. Upon achievement of a State license, the individual may be appointed on temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program.
