Position Overview
The Healthcare Coordinator plays a vital role in supporting behavioral health patients by helping them access the care, resources, and services they need to be successful. This individual serves as a liaison between patients, providers, and community organizations while coordinating referrals, assisting with insurance and assistance programs, maintaining documentation, and ensuring continuity of care. This is an excellent opportunity for someone passionate about patient advocacy, community health, and improving access to care for underserved populations.
Key Responsibilities
Coordinate referrals, appointments, screenings, and follow-up services for patients.
Connect patients with community resources, social services, and healthcare programs.
Assist patients with accessing Medicaid, KanCare, and other eligibility-based assistance programs.
Monitor patient progress and identify barriers to treatment or care.
Maintain accurate patient records, documentation, and care management files.
Verify demographics, insurance information, and patient financial responsibility.
Build and maintain relationships with community organizations, referral partners, and healthcare providers.
Participate in community outreach activities, educational events, and partnership meetings.
Support behavioral health program initiatives, quality improvement efforts, and care coordination activities.
Ensure compliance with HIPAA and patient confidentiality standards.
Required Qualifications
High School Diploma or GED.
Bilingual in Spanish and English.
Experience in care coordination, patient services, healthcare administration, social services, community outreach, or a related field.
Strong interpersonal and communication skills with the ability to work effectively with diverse populations.
Ability to manage multiple priorities in a fast-paced healthcare environment.
Strong organizational skills and attention to detail.
Proficiency with documentation, recordkeeping, and basic computer applications.
Bachelor's degree in Healthcare, Social Work, Psychology, Human Services, Public Health, or a related field preferred.
Experience working in a healthcare, behavioral health, community health, or nonprofit setting preferred.
Knowledge of Medicaid, community resources, and patient assistance programs preferred.
Experience supporting underserved or vulnerable populations preferred.

Pay and Benefits
The pay range for this position is $16.00 - $20.00/hr.
