Responsibilities The primary responsibility of the Registered Nurse - Survivorship Care Coordinator is to coordinate the journey of the veteran patient with cancer through the health care system. The incumbent of this position is essential to the successful operation and outcomes of the cancer treatments received in the Hematology/Oncology Department. The incumbents' activities are dedicated to the timely access to quality care and treatment for patients with cancer which increases patient safety and satisfaction. The nurse is a stable and reliable point of contact for Veteran patients, providers, and member of the clinical team. The incumbent provides oversight of the patient's journey through the diagnostic and treatment continuum, ensuring that all steps in the process are scheduled, and completed timely. Incumbent is responsible for tracking the overall treatment plan while following select patients from the earliest suspicion of cancer through the continuum of cancer care inclusive of diagnosis, treatment, survivorship/surveillance and or palliation/end of life. Typical procedures include PET scanning or other designated Nuclear Medicine procedures, radiology or laboratory studies; pre-operative diagnostic studies coordinated with clinical subspecialties such as Surgery, Cardiology or Dental Service; and /or facilitating clinic appointments, coordination and arranging diagnostic endoscopies or other invasive procedures. Facilitates seamless transition through multidisciplinary/departments throughout veterans' course of treatment. Demonstrate knowledge and ability to apply evidence-based practice to plan of care. Provides support at the unit/service and organizational level. Coordinates with laboratory and assisting in bone marrow biopsies; providing patient education ensuring informed consent and providing oversight to ensure completion of require clinical and/or administrative documentation in accordance with (VA) medical center policies and Joint Commission Standards.
