As a Nurse Practitioner (VSP), you will apply your advanced education and clinical competencies to achieve optimal patient outcomes. You will work on problems of diverse scope and complexity ranging from moderate to substantial.
You will work as a variable service provider NP/PA (APP) role with an average of 20-30 hours per week for a 6-month period. You will work in a remote environment, reporting to our Associate Director, Advanced practice Provider. There are no benefits tied to this position.
You will spend your day performing the following duties:
Conducting virtual (audio and video) visits with CenterWell patients to care for chronic conditions, discuss open care gaps, and connect patients back to an in-person PCP.
Address acute and chronic conditions and preventive care needs. Creating and registering, and documenting a patient record within Athena
Verify benefits
Comprehensive patient evaluation including medication and medical history review. Coordinating management of members social needs to help facilitate gap closure - e.g., providing transportation services
Ensure adequate follow up regarding receipt of results, medication initiation, and PCP follow up
To be successful in this role, you will have strong communication and interpersonal skills, allowing you to build collaborative relationships with patients, families, and care teams. You will excel in an integrated, team-based care environment, adapt to value-based care practices, and remain focused on delivering high-quality outcomes. Required Qualifications:
You will have a Master's degree or higher
You will hold a certification by the American Nurses Credentialing Center (ANCC) or American Academy of Nurse Practitioners Certification Board (AANPCB
You will have an active, unrestricted NP license in 2-3 or more of the following states: FL, TX, AZ, GA, LA, NC, SC, MS, VA prior to the start of employment
Preferred Qualifications:
Previous experience in a value-based care or Medicare Advantage setting
Knowledge of HEDIS measures and quality indicators
Experience conducting annual wellness or developing comprehensive care plans for patients who may only be seen once
Experience in virtual care and remote work

Pay Range
The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
$149,800 - $206,100 per year
