The Director of Health Home Care Management Services, reporting to the Vice President of New Initiatives, provides strategic and operational leadership for the Care Management Department, ensuring the delivery of high-quality, person-centered services in compliance with New York State Department of Health (DOH), Health Home, Medicaid, and organizational standards. This executive leadership position is responsible for driving strategic initiatives, operational excellence, and continuous quality improvement while advancing the organization's mission of delivering exceptional care management services. The position oversees departmental operations, quality improvement initiatives, regulatory compliance, fiscal performance, business development, and leadership of departmental staff while fostering a culture of collaboration, accountability, and continuous improvement.

Key Responsibilities

Provide strategic leadership and oversight of Care Management programs and operations.

Ensure compliance with Health Home, DOH, Medicaid, and agency regulations.

Supervise, coach, and evaluate management and departmental staff.

Lead quality improvement initiatives and partner with Quality Assurance & Improvement to monitor program performance.

Develop and implement departmental policies, workflows, and best practices.

Monitor key performance indicators (KPIs), prepare performance reports, and implement corrective action plans when needed.

Collaborate with the Fiscal Department on budgeting, productivity, and financial performance.

Build and maintain relationships with hospitals, managed care organizations, community providers, and referral partners to support program growth.

Promote interdisciplinary collaboration and integrated care for high-risk populations.

Support staff development through ongoing training, coaching, and performance management.

Qualifications

Bachelor's degree in Social Work, Human Services, Public Health, Healthcare Administration, Nursing, or a related field required. Master's degree in Social Work (MSW), Public Health (MPH), Healthcare Administration (MHA), or a related discipline preferred.

Minimum of seven (7) years of progressive Care Management experience serving vulnerable or high-risk populations.

Minimum of three (3) years of supervisory or management experience in Care Management or Health Home programs.

Strong knowledge of Health Home, Medicaid, and Department of Health regulations.

Demonstrated leadership, regulatory compliance, quality improvement, business development, project management, and relationship-building skills.

Proficiency with Electronic Health Records (EHRs), care management platforms, and Microsoft Office Suite.
