Case Manager (Adult - Behavioral Health)

JOB REQUIREMENTS: Responsible for planning, providing, arranging, and monitoring services to consumers participating in Comprehensive Community Services (CCS), Coordinated Services Team (CST) program, and to consumers who are dually= enrolled in Childers\'s Long Term Services. (CLTS) under the close supervision of the Division of Community Programs Program Supervisor. Essential Duties: Perform clinical assessments including initial intake and yearly reviews. Develop, direct and implement treatment plans, interventions, and crisis plans using a trauma-informed and person-centered approach. Ensure that the member receives culturally, linguistically, and developmentally appropriate and timely manner. Provide information, outreach, and assistance to youth and their families. Refer consumer to contracted services and monitor to ensure the consumer can achieve the highest level of independent functioning. Provide individual and family psychoeducation and individual skill development in community-based and home settings. Encourage investment and participation of natural supports in recovery concepts and processes. Interface with healthcare providers and other social service agencies to coordinate services. Assist families with transition to adult services and connect them to resources related to needs. Function as part of treatment teams/multi-disciplinary teams to ensure continuity of care. Communicate effectively through verbal, non-verbal, and written communications. Continually seek to improve skills and demonstrates a high level of self-direction. Maintain accurate and timely documentation in the form of assessments, treatment plans, progress notes, etc. as required by agency policy and State statute requirements including the authorizing and distribution of funds when warranted. Attend and participate in agency staffing and in-services. Participate in one on one, side by side, and group supervision. Other duties as assigned, including serving in a coverage role for other department staff. ** OTHER EXPERIENCE AND QUALIFICATIONS: Knowledge of applicable federal laws, Wisconsin Statutes, administrative rules, court procedures, and funding requirements governing Child Protective Services and related social services programs. Knowledge of child welfare practice, including child safety assessment, risk determination, family functioning, permanency planning, and court-involved case management. Understanding of social problems, cultural diversity, trauma, human growth and development, and individual and family dynamics as they relate to assessment, service planning, and intervention. Knowledge of or experience with trauma-informed principles and practices and the ability to apply those principles when working with children, families, caregivers, service providers, and community partners. Skill in receiving and responding to reports of child abuse or neglect and conducting safety, risk, and family assessments in accordance with statutory and policy requirements. Skill in developing, implementing, and monitoring safety plans, case plans, and permanency plans in collaboration with families, caregivers, and multidisciplinary partners. Skill in coordinating services and activities that cross unit, departmental, and agency lines, including collaboration with courts, law enforcement, schools, healthcare providers, and contracted service providers. Skill in establishing and maintaining effective working relationships with coworkers, supervisors, service providers, foster parents, community partners, and members of the public. Effective written and verbal communication skills, including accurate documentation, preparation of court reports, provision of written and oral testimony, facilitation of meetings, and explanation of program requirements, services, and client rights. Demonstrated commitment to confidentiality and ethical handling of sensitive information in accordance withlegal requirements, professional standards, and County policies. Sound professional judgment, emotional composure, and ability to de-escalate situations when responding to stressful, complex, or confrontational circumstances. Ability to work independently and collaboratively; follow instructions; manage multiple cases and deadlines; and complete assigned tasks accurately and timely.
