Overview

Helio Health's In Community Program (formerly Circare) is designed to help individuals and families, who have encountered certain obstacles develop the resources and supports to live a satisfying, naturally independent life.

TheAdult Health Home Care Coordinator works with and oversees the process with individuals to develop and maintain a comprehensive plan for accessing and using services and supports necessary for a person's health.

Pay Rate:$22.12 per hour

In addition to Helio Health's comprehensive benefits package, this position is eligible for a $1,500 Sign-On Incentive!

Responsibilities

Conduct outreach, and assessment for Health Home services for individuals who are Medicaid Eligible and/or have active Medicaid.

Conduct eligibility screenings and Health Home enrollment.

Collaboratively work with enrollee to create a comprehensive person-centered plan of care to meet their medical, behavioral health and social service needs.

Responsible for health promotion, education, and overall coordination of all aspects of an individual's care.

Offer family support, and referral/linkages to community, social, and other support services as needed.

Leads multidisciplinary teams of medical, mental health, chemical dependency treatment providers, social workers, nurses and other care providers to assuring enrollees receive services.

Assist individual in keeping appointments and monitor service delivery.

Provide Care Management services at various locations including in community locations and enrollees' homes.

Qualifications

Bachelor's degree required, preferably in Human Service, Forensic, Psychology, Public Health, or related concentration.OR

CASAC and two years' experience working with individuals with mental health, substance use or co-occurring disorders,required.

Must have a valid driver's license and own transportation:

Helio Health's insurance carrier requires that employees who drive personal vehicles for work maintain minimum auto insurance coverage limits of at least $100,000/person; $300,000/accident and that employees provide HR with proof of current minimum insurance coverage (copy of Auto Liability Insurance Declaration Page) on their first day of employment. Employees are required to provide an updated Auto Liability Insurance Declaration Page to HR by expiration date on Declaration Page.

Multilingual a plus.

Salary and hourly compensation ranges are provided in accordance with NYS law and are based on Helio Health's good faith belief of what is accurate at the time of posting. Salary and hourly compensation offers are based on candidate's education level and experience relevant to the position and also take into account information provided by the hiring manager and program.
