Summary This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. This may involve use of the OEF/OIF Department of Defense (DOD) Registry, Veterans Information System (VIS), Patient Treatment File searches, Veterans Health Information System and Technology Architecture (VistA), Care management Tracking and Reporting Application(CMTRA) and Computerized Patient Record System (CPRS) records, Northeast Program Evaluation Center (NEPEC) data reports and/or Classification reports, Patient Data Exchange, Network Health Exchange, and contacts within the community. Ensures that severely-injured and/or ill post 9/11SM/Vs and other SM/Vs with identified case management needs are assigned to a nurse or social worker case manager and oversees or works closely with care management teams to include nurse and social worker case managers, the Visual Impairment Services Team (VIST) Coordinator, the Spinal Cord Injury (SCI) Coordinator, the Polytrauma Case Manager if applicable, and the Women Veterans Program Manager. Serves as the facility point of contact for all inquiries regarding transition of post 9/11 SM/Vs and as point of contact for post 9/11SM/Vs seeking health care from the facility. These duties include a wide variety of stakeholders (e.g., Veterans, families, state and community agencies, Veterans Service Organizations, military support programs, etc.). Incumbent addresses Congressional inquiries and Veterans Integrated Service Network (VISN) and Department of Veterans Affairs (VA) Central Office action items as requested. Coordinates comprehensive program elements and services for preventive mental and physical health evaluations for program participants. Promotes and coordinates evaluations for the M2VA/TCM Program anywhere within the facility and any outlying Community-based. Prepares program reports for the VA Central Office, VISN POC, and Medical Center leadership. Monitors procedures for referrals between the VA medical center, Military Treatment Facility and/or community hospitals that promote appropriate and timely inter-facility transfers. Responsible for management of the Military Treatment Facility referrals and other identified post 9/11SM/Vs located in the catchment area. Ensures registration of appropriate Veterans for clinical, administrative, and outcome purposes. Collaborates with Enrollment and Eligibility staff to ensure access to care. Establishes performance and outcome standards for the facility M2VA/TCM Program that promote quality and efficiency of services in coordination with the overall goals of the medical center and those of the VISN and VA Central Office. Grade Determinations GS-12 Supervisory Social Worker: Education and Experience: One year of experience equivalent to the GS-11 grade level. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as Supervisory Social Workers must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities: In addition to the experience above, the candidate must demonstrate all of the following KSAs: Ability to independently organize work, set priorities, and meet multiple deadlines. Knowledge of supervisory duties which includes clinical supervision, consultation, negotiation with other departments and quality improvement.
