The Nurse Case Manager supports care coordination, administrative nursing functions, and operational workflows within a provider or payer setting. Working under the supervision of the Clinical Director, this non-clinical, non-patient-facing role focuses on facilitating communication, supporting care programs, and contributing to process improvement initiatives aligned with organizational goals. The position operates within a team-based environment that supports population health and internal care processes, emphasizing coordination, documentation, and collaboration rather than direct patient care or clinical decision-making.
Responsibilities
Coordinate care management activities through administrative support and structured workflows.
Collaborate with providers, care teams, and external partners to support continuity of care processes.
Review and manage documentation, referrals, and care plans based on established guidelines.
Provide general health education and resource information using approved materials.
Route patient and provider inquiries to appropriate clinical or operational teams.
Support workflow efficiency, scheduling alignment, and internal processes.
Serve as a resource to internal teams on systems and coordination workflows.
Contribute to quality improvement initiatives and reporting.
Assist with onboarding coordination, scheduling, and team support functions.
Participate in internal meetings and organizational initiatives.
Perform additional administrative duties as assigned.
Maintain compliance with all organizational policies and procedures.
Maintain regular attendance and punctuality to support consistent operations.
Essential Skills
Active, unencumbered Registered Nurse (RN) license, with Kansas licensure preferred.
Strong communication skills, with the ability to convey information clearly to providers, internal teams, and external partners.
Proven coordination and organizational skills to manage multiple tasks, documentation, and workflows.
Ability to work effectively in a structured, fast-paced environment.
Strong time management and problem-solving abilities.
Cultural awareness and the ability to support diverse populations.
Foundational understanding of care coordination and utilization review support.
Knowledge of healthcare operations in provider or payer settings, or population health, preferred.
Comfort using healthcare systems such as electronic medical records (EMR/EHR) and other administrative tools.
Additional Skills & Qualifications
Graduate of an accredited Registered Nurse (RN) program.
2+ years of experience in care coordination, case management, utilization review, or healthcare operations preferred.
Experience in payer, outpatient, or administrative healthcare environments preferred.
Prior leadership or team support experience is a plus.
Experience working in health care, patient care, or insurance-related environments is beneficial.
Ability to contribute to process and quality improvement initiatives within healthcare operations.

Work Environment
This is an on-site, office-based role within a provider or payer organization, operating Monday through Friday from 8:00 AM to 5:00 PM. The environment is fast-paced and team-oriented, with a strong focus on coordination and administrative support for population health and internal care processes. The role involves heavy use of computers, phones, and healthcare systems such as EMR/EHR platforms, along with frequent collaboration with internal teams, providers, and external partners. There is no direct patient care, no performance of clinical procedures, and no exposure to clinical treatment environments. Work follows structured workflows and clearly defined performance expectations that are aligned with operational goals. Pay and Benefits
The pay range for this position is $25.00 - $35.00/hr.
