Summary At Sioux Falls VA Health Care System, Certified Foot Care Nurse (CFCN) Registered Nurse (RN) care managers partner with Veterans and families to coordinate care and ensure accurate documentation-empowering informed decisions and timely, high-quality services across settings. Responsibilities The VA Midwest Health Care Network advocates for a Whole Health System of care in each of the Medical Centers. Demonstrate knowledge and skills in foot care, including, but not limited to: toenail care (including use of Dremel), basic and intermediate foot care, a variety of wound dressing changes related to co-morbidities and risk factors, identify potential for skin breakdown and intervene with appropriate preventative measures, collect pertinent data, ongoing monitoring and documentation including pressure, surgical, vascular, and diabetic/neuropathy wounds, and use of nurse protocols. Participating in performance improvement; identifying opportunities to improve patient care through monitoring, analyzing, and evaluating care outcomes. Exhibit leadership in assisting other staff to assess and evaluate patients foot skin integrity and wounds. Communicating effectively with internal and external customers and collaborating with facility and Community-Based Outpatient Clinics to ensure continuity of care.
