Job Summary
The Transplant Clinic ACU is a multidisciplinary ambulatory care unit dedicated to providing high-quality, patient-centered care to solid organ transplant patients across the continuum of care. The clinic supports patients undergoing evaluation, transplant, and long-term post-transplant management while partnering closely with adult and pediatric specialty teams to optimize patient outcomes and experience.
The Transplant Clinic ACU supports multiple transplant programs including kidney, liver, lung, heart, and living donor services and works collaboratively with providers, nursing, allied health professionals, and operational teams in a fast-paced and highly specialized environment. The clinic is committed to operational excellence, compassionate care, teamwork, and continuous process improvement while supporting the complex needs of transplant patients and families.
We are seeking compassionate, adaptable, and team-oriented individuals committed to providing exceptional patient care and operational support. This position will partner closely with Pediatric Cardiology, Pediatric Heart Transplant, and Transplant Operations teams to support the pediatric heart transplant patient population.
This position is hybrid, with the expectation that the Medical Assistant will be onsite a minimum of two times per week to support clinic operations, direct patient care activities, and onsite responsibilities.
Responsibilities*
Provides patient intake and prepares patients for clinic visits, examinations, and procedures while ensuring excellent customer service and efficient patient flow for the pediatric heart transplant population. Responsibilities include both onsite and virtual patient support within scope of practice and departmental guidelines.
Patient Intake:
Responds to general patient and family inquiries regarding clinic operations within scope of practice.
Rooms patients virtually and in person and performs standard patient intake including vital signs, medication reconciliation, medical history updates, screenings, documentation, and other assigned tasks following established workflows and protocols.
Prepares and reviews patient information prior to appointments, including medical records, prescription updates, and relevant clinical documentation.
Performs room preparation and turnover duties, ensuring exam and clinic spaces are stocked, organized, and prepared for patient care activities.
May assist with obtaining outside records, diagnostic results, and supporting documentation necessary for transplant-related visits and coordination of care.
Patient Care:
Supports direct patient care activities within competency and Ambulatory Care guidelines.
Administers medications, injections, and immunizations per standing orders, clinic protocols, and competency requirements.
Performs point-of-care testing, EKGs, specimen collection, and assists providers with basic treatments, tests, and procedures as appropriate.
Enters orders per policy and queues medication renewals within scope and established workflows.
Documents patient information accurately in MiChart within scope of practice.
Observes patient status and reports changes appropriately to providers and clinical staff.
Acts as a chaperone during sensitive exams, diagnostic procedures, or upon patient request.
Provides routine patient instructions and assists with forms and consent workflows as assigned.
Supports transplant-specific workflows including coordination of clinic readiness, patient follow-up tasks, and communication with interdisciplinary care teams.
Administrative and Clinical Workflow Support:
Efficiently prepares and organizes clinic and procedure spaces with appropriate equipment and supplies.
Maintains and cleans clinical equipment according to department and infection prevention guidelines.
Supports hybrid clinic workflows by collaborating with Pediatric Cardiology, Pediatric Heart Transplant, and Transplant Operations teams.
Assists with coordination of care activities, patient follow-up tasks, inbox management support, and operational workflows supporting the pediatric heart transplant population.
Maintains clinic supply inventory and assists with stocking and operational readiness of clinical areas.
Contributes to a safe, respectful, and supportive environment for patients, families, faculty, and staff by following established policies, procedures, and guidelines.
Participates in departmental training, competency validation, and process improvement initiatives as assigned.
Required Qualifications*
High school diploma or GED.
Graduation from a medical assistant program or an equivalent combination of education andexperience (3 - 4 years).
Knowledge of basic medical terminology.
Experience in panel management including provider registry data and exception reports
Desired Qualifications*
Additional formal education in a healthcare field.
Certification as a Medical Assistant
Previous ambulatory care or transplant experience preferred.
Pediatric experience preferred.
Experience working in a highly specialized, multidisciplinary clinical environment preferred.
Familiarity with MiChart/Epic preferred.
Excellent interpersonal, communication, and organizational skills.
Ability to work collaboratively in a multidisciplinary clinical environment.
Demonstrated ability to prioritize work and adapt in a fast-paced ambulatory care setting.
Experience with electronic medical records preferred.
Ability to support both onsite and remote operational workflows in a hybrid work environment.
