This Licensed Practical Nurse (LPN) role is an integral part of a collaborative clinical team, responsible for delivering safe, evidence-based patient care while providing exceptional support to both patients and clinicians. You will anticipate patient and provider needs, contribute to a respectful and organized care environment, and help ensure that every patient receives high-quality, compassionate service.
Responsibilities
Monitor, document, and communicate patients' vital signs, measurements, and clinical status in a timely and accurate manner.
Observe patients closely and promptly note any physical or emotional changes, escalating concerns to appropriate clinicians as needed.
Prepare examination rooms for patient visits, ensuring all necessary equipment, instruments, and supplies are available and functioning properly.
Select, prepare, and manage medical supplies and equipment to support efficient and safe patient care.
Maintain a comfortable, organized, safe, and hygienic environment for patients at all times.
Maintain accurate and up-to-date records of patient information in the electronic medical record (EMR) system.
Prepare patients for follow-up care after treatment by providing clear instructions and ensuring understanding of care plans.
Potentially guide, support, and educate other LPNs, contributing to a collaborative and knowledgeable nursing team.
Serve as a knowledgeable resource on nursing-related issues within the clinical section, supporting both patients and colleagues.
Organize and manage scheduling for patients and any necessary medical tests or procedures to support smooth clinic operations.
Essential Skills
Active Licensed Practical Nurse (LPN) license in the state of New York.
2+ years of LPN experience in a clinical or healthcare setting.
1-2 years of medical office experience, including direct patient care and nursing clinical duties.
Proficiency with electronic medical records (EMR) systems.
Strong patient care skills with a focus on safety, evidence-based practice, and compassion.
High level of attendance, reliability, and professionalism.
Ability to observe and recognize changes in patients' physical and emotional status and communicate effectively with the care team.
Organizational skills to manage exam room readiness, supplies, and patient scheduling efficiently.
Additional Skills & Qualifications
Basic Life Support (BLS) certification is a plus.
Experience in a community health center or outpatient medical office environment is beneficial.
Ability to educate and support other LPNs and team members when needed.
Strong interpersonal and communication skills to interact effectively with patients, families, and clinical staff.
Commitment to providing respectful, high-quality care to diverse patient populations, including underserved and vulnerable communities.
Work Environment
This position is based in a community health center setting that offers comprehensive primary, preventive, and behavioral health services to a broad and diverse patient population, including underserved and vulnerable communities. You will work with a multidisciplinary team of clinicians and staff, using electronic medical record (EMR) systems and standard medical office equipment in exam rooms and clinical areas. The schedule is primarily Monday through Friday, with a regular daytime schedule that includes 1-2 late evenings per week, during which the clinic remains open until around 7 p.m. on those days; start times on late days are adjusted later in the morning. The role also includes rotating weekend shifts. The environment is fast-paced yet supportive, with a strong focus on teamwork, patient-centered care, and maintaining clean, safe, and organized clinical spaces. Attire is typical professional clinical wear appropriate for a medical office setting.

Pay and Benefits
The pay range for this position is $28.00 - $35.00/hr.
