Responsibilities:
Provide direct care to residents, including assistance with activities of daily living (ADLs), such as bathing, dressing, and grooming
Monitor and report changes in resident condition to licensed nursing staff
Maintain a safe and clean environment for residents, including supporting housekeeping and maintenance tasks
Collaborate with interdisciplinary teams to develop and implement individualized care plans
Communicate effectively with residents, families, and colleagues to ensure seamless care transitions
