This position provides subject matter expertise to the Center for Health Promotion in advancing evidence-based and promising practices to improve identification and management of cardiometabolic conditions in rural settings. It leads statewide planning, coordination, and implementation of Rural Health Transformation Program initiatives that drive best practices and health systems change to improve population health outcomes in rural Minnesota communities.
The role focuses on designing and implementing innovative, evidence-based strategies to reduce risks, complications, and barriers to the prevention and control of cardiovascular disease. Clinical areas include cardiovascular disease, stroke, hypertension, dyslipidemia, diabetes, pre-diabetes, and chronic kidney disease.
Located within the Cardiovascular Health Unit of the Health Promotion and Chronic Disease Division, this position partners with the Office of Rural Health & Primary Care and collaborates across CDC-funded and chronic disease programs to promote wellness, prevent disease, and improve cardiovascular health outcomes statewide.
Primary responsibilities include:
Provide statewide planning, consultation, and technical assistance to support the implementation of evidence-based and promising practice models to improve chronic disease prevention, identification, and management, with a focus on cardiometabolic conditions (heart disease, stroke, diabetes, and chronic kidney disease), cancer screening and follow-up, and post-discharge supports for patients with cardiometabolic conditions in rural settings.
Provide leadership and support to MDH teams on statewide planning, consultation, and technical assistance to support the implementation of evidence-based and promising practice models to link patients from clinical settings to community-based support services.
Serve as a liaison between ORHPC, HPCD programs (including Center for Health Promotion), advisory groups, and contracted technical assistance providers. Document and align RHTP-implemented programs with existing Minnesota initiatives, programs, and services to support successful statewide implementation.
In accordance with HR/LR Policy #1422 Telework, this position is eligible for up to 50% telework for applicants who live in Minnesota or in a bordering state within 50 miles of the assigned work location. Candidates residing outside of Minnesota further than 50 miles of the assigned work location at the time of application must be able to relocate to Minnesota within 30 days of their start date. The telework policy may exempt employees who live more than 50 miles from their primary work location from the 50% in-office requirement, depending on job requirements, helping the State of Minnesota access top talent across the state.
MDH relies heavily on grant funding, in addition to state appropriated dollars. As an agency, we continually monitor funding levels and navigate some degree of uncertainty related to grant cycles and the impact of the grant funding on program priorities.
This position is currently funded and approved to be filled. While the position is funded at this time, future funding is dependent on the continuation of our funding sources. The Minnesota Department of Health works proactively to sustain programs and maintain our workforce, to avoid reductions in force to the very best of our ability. In the event of a loss of funding resulting in a necessary workforce reduction, that process is governed by our collective bargaining agreements and compensation plans.
Minimum Qualifications
Three (3) years of experience in healthcare systems, public health, or community health programs. Experience must include all of the following:
Experience leading or coordinating projects or initiatives, including developing work plans, timelines, or deliverables.
Experience working within healthcare organizations (e.g., hospitals, clinics, FQHCs).
Experience working within rural settings.
Experience managing or coordinating work with partners, such as external organizations, grantees, contractors, or cross-functional teams.
Experience implementing programs or initiatives using established frameworks, best practices, or evidence-based approaches.
A bachelor's degree in public health, health administration, nursing, social work, or a closely related field may substitute for one (1) year of experience. A master's degree may substitute for eighteen (18) months of experience.

Preferred Qualifications
Experience working in or with rural health systems and/or Tribal Nations, including understanding of rural healthcare delivery challenges and community-clinical partnerships.
Experience supporting or leading grant-funded initiatives (e.g., CDC, HRSA), including program implementation, reporting, and working with grantees or contractors.
Experience implementing or advising on clinical quality improvement, health systems change, or evidence-based interventions related to chronic or cardiometabolic conditions.
