Summary This Advanced Practice Nurse position is within the Specialty Care Services at the Connecticut VA Healthcare System located in West Haven, CT. It is permanent and full-time at 40 hours per week. Responsibilities The Advanced Practice Nurse (Nurse Practictioner) provides a full range of services emphasizing quality, timeliness and the advancement of the VA mission. The NP provides advanced clinical practice, consultation and management. Duties include, but are not limited to: Manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice; Develops and implements an individualized plan to achieve optimal population health and disease management; Demonstrates leadership and works collaboratively with Primary Care Team and other interdisciplinary team members; Orders and interprets diagnostic studies; Prescribes pharmacologic and non-pharmacologic interventions; Initiates referrals/consults to specialty services; Provides education to patients and their families; Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes; Exemplifies the ICARE core values of-Integrity, Commitment, Advocacy, Respect, and Excellence; and Performs other duties as assigned. Qualifications Advanced Practice Nurse (Nurse Practitioner) Basic Requirements Education: A post-master's certificate or master's or doctoral degree as a Nurse Practitioner. (Applicants pending the completion of educational requirements may be referred and tentatively selected but may not be hired until the education requirements are met.) Must be a graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). NOTE: Foreign Education. Graduates of foreign schools of professional nursing programs, the nurse practitioner degree equivalency must be validated through an Agency recognized degree equivalency evaluation agency, such as the International Consultants of Delaware (ICD). Candidates with equivalent nurse practitioner masters or doctorate level degrees who possess current, full, active, and unrestricted registration and certification will meet the requirement of graduation from an approved school of professional nursing to warrant appointment. License - All APN (NPs) must possess and maintain an active, current, full, and unrestricted APN license to practice as an NP in a State, Territory or Commonwealth of the United States, or in the District of Columbia. Demonstrates effective communication skills and professional behavior, promoting cooperation and teamwork Preferred Experience: Demonstrated experience reviewing and triaging specialty GI endoscopy consults, including evaluation of procedural indications, anesthesia requirements, and periprocedural anticoagulation considerations within a high volume, open access clinic environment; Experience performing and/or assisting with specialized gastroenterology diagnostic procedures, such as obtaining informed consent and administering capsule endoscopy, and assisting with manometry catheter placement; Skill in managing complex enteral access (PEG) patients, including readiness assessment, interdisciplinary coordination (oncology, primary care, nutrition, ALS clinic), post procedure follow up, and troubleshooting of PEG related complications; Proven ability to lead or support gastroenterology quality improvement initiatives, such as H. pylori eradication stewardship, Barretts esophagus management, and optimization of bowel preparation workflows for complex cases; Experience serving as a primary clinical point of contact for patients and providers, responding to clinical queries related to endoscopy results, consult status, and bowel prep challenges, while ensuring timely and accurate documentation across clinic, telephone, results, and patient portal (MHV) encounters; and Demonstrated proficiency using clinical technologies and population health tools, including dashboards, disease management platforms and other systems that support efficient care coordination and multidisciplinary communication.
