Job Summary
The C.S. Mott Children?s Hospital is hiring a part time Nurse Practitioner to become part of the hospital/community program Partners for Children-Little Heroes League. This program provides home and community based services for children with chronic complex medical conditions who receive specialty and hospital based care at C.S. Mott Children?s Hospital. The NP will perform assessments and care management visits in the patient?s home, join specialty clinic appointments, and complete virtual video visits.
Responsibilities*
Under physician supervision, coordinates care of chronic, complex infants, children and adolescents to children age 25
Specifically, role requires the NP to perform patient assessments at intervals of every two (2)weeks to two (2) months via at-home, telehealth, clinic, or in-patient visits, to gather information obtained from specialty visits, primary care visits, and hospital encounters and to create and update records that consolidate case summaries, problem lists, and medication lists
Create and update care plans, under physician supervision.
Conduct and document yearly comprehensive assessments on patients
Participate in patient/family/provider/school meetings as an advocate for the patient when needed
Participate in bi-weekly interdisciplinary team meetings, and weekly rounds to discuss new and active patients
Communicate assessments and care plans to the primary care provider and specialty consultants
Order tests and prescribe medications
Confer with attending physicians and communicate with other care providers/consultants to coordinate clinical care
Maintain accurate documentation regarding patient status, evaluations, and treatment
Perform other related duties as requested by the attending physician
Utilize new improvements and/or technologies that relate to job assignment
Required Qualifications*
Required Qualifications NP
Current licensure to practice as a Registered Nurse and Nurse Practitioner in the State of Michigan
National Certification as a Pediatric Acute Care Nurse Practitioner or Pediatric Primary Care Nurse Practitioner
Certification as a Nurse Practitioner recognized by the National Council of State Boards of Nursing (NCSBN).
Graduation from an accredited Nurse Practitioner program.
3 years of direct patient care experience caring for medically complex children with advanced illnesses and their families or children receiving palliative services
Current DEA registration
AHA Approved BLS
Required Qualifications PA
Current license to practice as a Physician Assistant in the State of Michigan.
Satisfactory completion of a physician assistant program accredited by ARC-PA or one of its predecessor agencies.
Certification by the National Commission on Certification for Physician Assistants (NCCPA).
3 years of direct patient care experience caring for medically complex children with advanced illnesses and their families or children receiving palliative services
State of Michigan Controlled Substance License Required
Current DEA registration
AHA Approved BLS
NOTE: Employment is contingent upon successful completion of the credentialing and privileging process.

Desired Qualifications*
2-years experience as a practicing Advanced Practice Professional.
Experience coordinating care of complex patients with chronic illnesses.
Experience caring for children with high symptom burden and/or complex conditions requiring multiple consultative services.
Advanced Practice Hospice and Palliative Care Nurse Certification and/or Palliative Care training program.
Demonstrated ability to write and communicate clearly
Demonstrates personal and professional integrity; collaborative and respectful behavior; flexibility

Work Schedule
Hours/Week: 20 Hours/Week
Shift/Hours/Days:. 0.5 FTE, day shift
NPs will take call on average of one (1) night and one (1) weekend per six (6) week period. Weekend and holiday coverage may be required.
Union Affiliation
This position is covered under the collective bargaining agreement between the U-M and the United Physician Assistants of Michigan Medicine, which contains and settles all matters with respect to wages, benefits, hours and other terms and conditions of employment.
