Job Purpose: The Certified Nursing Assistant/Driver for the Pediatric Skilled Nursing functions in a dual role as a member of the patient care team. As a certified nursing assistant this role performs performing basic caregiving tasks within the scope of certification and demonstrated competence and performs nursing tasks delegated by and under the direction and supervision of a registered nurse or other licensed personnel. As a driver, this role is responsible for safely operating a vehicle (van or minibus) to pick up and drop off children enrolled in the program.

Responsible for the supervision and safety of clients in a multi discipline care setting.
Required to observe client activities, detect areas of danger, and distinguish when clients who are non-communicative are in need of personal cares, and converse clearly with clients and staff on a frequent basis.
Performs basic personal care tasks such as bathing and mouth care, grooming, dressing, toileting, eating assistance, and skin care.
Provides basic nursing skills within level of competency and as delegated by a licensed nurse. i.e., hygiene, physical cares, intake/nutrition including GT and JT feed administration and light meal. preparation, ambulation/exercise, maintaining infection control and safety, assists with tracheostomy and GT stoma cares.
Provides basic restorative services including the application of assistive devices for ambulation, range of motion exercises, proper turning and positioning in bed and chair, bowel and bladder training and use and care of prosthetic devices.
Works collaboratively with childcare staff to meet all aspects of developmental and academic needs of the child. Assists teacher, other staff, and clients to participate in and engage clients in client activities.
Accompanies the children on field trips and outdoor play as needed.
Performs light housekeeping tasks, i.e., toy cleaning, laundry, dishwashing, etc.
Cleans equipment and supplies after use and reports any deficiencies or unsafe equipment to the RN in charge or appropriate personnel immediately.
Communicates effectively with clients and families, assures confidentiality of all patient/family information.
Operate Pediatric Skilled Nursing (PSN) vehicles over designated routes or in response to dispatcher/supervisor instructions.
Comply with all driver and passenger record keeping requirements that include daily driver trip sheets, vehicle inspections, rider pickup and drop-off times, Title XIX (MTM Contract requirements) rider or physician documents plus parent or caregiver correspondence.
Aid clients / children with acute medical conditions when boarding and exiting vehicle.
Ensure the safety of all riders by use of their appropriate passenger restraining device inside the vehicle.
Provide minor vehicle maintenance when needed (replacement bulbs, checking fluid levels and tire pressure plus specified interval testing of all vehicle safety equipment and other approved tasks).
Readily adjust to variable working conditions including adverse weather, traffic passenger challenges.
Document all assigned vehicle activities for each day including odometer readings, fuel purchases, vehicle deficiencies as well as any other required information.
Maintain the vehicle's interior and exterior appearance.
Provide secondary transportation to other designated agency divisions for medical and social trip purposes.
Attend departmental and agency-wide safety briefings and training sessions (both formal and informal) as required.
Works in partnership with the quality department to develop appropriate measures in support of program outcomes.
Participates in department quality planning activities on continuous improvement, logic models, performance metrics.
Other duties as assigned.
Qualifications
Required Education, Experience, Certifications, Licensure and Credentials: (Where appropriate, education and/or experience may be substituted)
Minimum Required Education : High School Diploma or equivalent.
Minimum Required Experience : Experience with pediatric special needs clients is preferred.
Required License - Certification - Registration: Issued By Governing Body (if applicable):
Current WI Nursing Assistant License, without limitations that are incompatible with duties, responsibilities, supervision or practice setting.

Required Auto Insurance : Vehicle liability insurance in accordance with Agency policy
Knowledge, Skills, & Abilities:
Must either be CPR certified at the start of employment or complete CPR certification course within 30 days of the start of employment. Must maintain active CPR certification through periodic renewal as dictated by the certification.
Ability to successfully complete all required driver trainings within 90 days of hire to include Defensive Driving, First Aid, CPR, vehicle evacuation procedures, children's mobility and securement devices, two-way communication procedures, Passenger Assistance and addressing bio-hazard situations.
Able to respond to emergency situations in accordance with training received.
Able to understand and follow written and verbal directions and speak and write in a clear concise manner.
Accurately and promptly communicate PSN Participant or parent/caregiver concerns to designated PSN staff.
Ability to read and understand a map of the PSN service area (Milwaukee County) and utilize navigation tools if available.

Physical Requirements, Visual Acuity, and Work Conditions:
Physical Requirements: Must frequently be able to move, transport, position, push/pull and/ or lift objects or equipment weighting at least 50 pounds a distance of 450 feet. Ability to successfully deal with change and stressful situations. This position is responsible for the supervision and safety of clients in a multi discipline care setting. As such, this position must be able to observe client activities, detect areas of danger, and distinguish when clients who are non-communicative are in need of personal cares, and converse clearly with clients and staff on a frequent basis.
Must be able to safely push a person in a wheelchair ramp in various weather conditions. Must be able to transfer a person from a wheelchair to the toilet, chair, or van seat/car with use of a gait belt and/or manual or power Hoyer lift.
Visual Acuity: The worker is required to have close visual acuity to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal.
Working Conditions: This position may work in a variety of indoor and outdoor work environments which may include, various outdoor weather conditions, loud noises, and fluctuating temperatures and the occasional use of stairs. PSN operates from 6am to 6pm 5 days per week and requires flexibility to cover the needs of the business as it evolves and changes.
