How you'll make an impact in this role

Improves patient safety by collecting comprehensive and accurate medication histories at admission, reducing the risk of medication discrepancies, adverse drug events, and preventable harm during care transitions.

Supports clinical teams by freeing up physicians and nurses from time-intensive medication reconciliation tasks, allowing them to dedicate more attention to direct patient care and acute clinical decision-making.

Reduces hospital costs and readmission rates by identifying and resolving medication errors early in the admission process, minimizing the downstream clinical and financial impact of drug-related complications. Strengthens care coordination across departments by serving as a dedicated liaison between patients, caregivers, pharmacies, and clinical staff to ensure accurate and up-to-date medication information is available at every stage of the patient's hospital stay.

What minimum requirements you'll need

Licensure / Certification / Registration:

Pharmacy Tech credentialed from the Pharmacy Technician Certification Board (PTCB) preferred.

Pharmacy Tech credentialed from the National Healthcareer Association preferred.

Education:

High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.

What additional preferences we're seeking

Ability to work independently, stay focused and organized, and manage multiple tasks in a fast-paced Emergency Department environment with shifting priorities.

Ability to accurately record drug names, dosages, routes, and frequencies.

Experience identifying medication discrepancies and catching errors before they affect patient care.

Comfortable speaking with patients, family members, and caregivers in stressful or emotionally sensitive situations.

Willing to reach out to outside pharmacies, care facilities, or doctor's offices to confirm medication information.

Dedicated to patient safety and able to communicate clearly with patients who may be confused or have difficulty recalling their medications.
