The LPN is directly responsible to the Registered Nurse (RN) and/or the MD/DO for performing identified responsibilities within the scope of their education and employment, as well as for the quality of care provided and their personal and professional conduct. The LPN is accountable to the Nurse Manager or their designee for completing patient assignments and for effectively communicating patient responses to the designated RN and/or MD/DO. Major duties of the Licensed Practical Nurse Patient Aligned Care Team, PACT position includes, but are not limited to: Provide direct patient care and assist with specialized nursing procedures in the ambulatory care setting. Collect, observe, and document patient data to support the development and implementation of care plans. Administer medications and treatments as prescribed and monitor patient responses. Recognize changes in patient condition and report significant findings to the RN or provider. Assist providers with diagnostic and therapeutic procedures. Participate in interdisciplinary team meetings and contribute to care planning for assigned Veterans. Educate patients and families regarding health conditions, medications, and treatment plans. - Maintain accurate documentation in the electronic health record. Ensure compliance with infection control, safety, and VA policies and procedures. Support team-based care by collaborating with healthcare staff to promote quality patient outcomes. Participate in quality improvement initiatives and maintain accreditation and regulatory readiness. Care Management: Carry out more specialized patient care duties, including assignments in selected, more complex inpatient and/or outpatient care settings. Possess comprehensive practical nursing knowledge and skills in specialized procedures and practices to serve as a resource person for other staff in organizing and implementing more complex/specialized patient care. Education - Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing Accrediting Commission (NLNAC) at the time the program was completed by the applicant. Exceptions include health care education in the military service or training in the military service which is accepted by the licensing body in the jurisdiction in which the individual is licensed as qualifying for full LPN/LVN licensure will be accepted as meeting the education requirements for VHA employment OR granted a license by a jurisdiction that does not require graduation from an approved school AND have at least one year of successful practice as an LPN/ LVN. GS-07: The GS-07 grade level is for select, complex LPN/LVN positions, as established by each health care facility. To be advanced, employees must meet all of the following standards, provided the standard is part of their assignment. Completion of at least one (1) year of additional qualifying experience at the GS-06 level or equivalent, fully meeting all of the performance requirements for the GS-06 LPN/LVN. Assists individuals or groups of patients and their families to take an active role in promoting health lifestyles for more positive outcomes. Exercises skill in influencing and communicating with unusually difficult to care for or communicate with patients and/or family members who may exhibit problems with lack of self-control, resistant or abusive behaviors, or impediments in their ability to understand or follow instructions. Skill, creativity, and initiative in contributing to the improvement of overall care delivery and patient and/or family satisfaction levels. Comprehensive practical nursing knowledge and skills in specialized procedures and practices to serve as a resource person for other staff in organizing and implementing more complex/specialized patient care. Ability to support the RN or MD/DO in monitoring the practice of unlicensed nursing or non-medical staff in patient care delivery Preferred Experience: The ideal candidate is a proficient professional nurse with a comprehensive background in cardiovascular care and device management. Preferred qualifications include: Proven track record: 2-3 years of dedicated LPN/LVN practice. Outpatient Expertise: Minimum of 1 year navigating the fast-paced environment of an outpatient clinic. The full performance level of this vacancy is GS-07. The actual grade at which an applicant may be selected for this vacancy is GS-07. Physical Requirements: The population of male and female veterans served ranges from the adult, age 18, to the elderly adult, age 65 and over. The incumbent must be physically and mentally able to perform the essential functions of the position without hazard to themselves or others .The incumbent must be able to demonstrate knowledge and ability to provide full range of complex technical nursing care to patients with a variety of physical and/or behavior problems. This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, pushing, use a keyboard, and perform CPR. Transferring patients and objects using appropriate assistive devices, push/pull an occupied gurney may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be able to perform all other activities of the position without restrictions.
