Your Role & Responsibilities
You will provide comprehensive care to seniors following clinical standards.
You will collaborate daily with your care team to ensure coordinated, high-quality care.
You will manage referrals, hospital/SNF coordination, DME, and home health services.
You will participate in daily huddles.
You will document care accurately with support from quality-based coders.
You will participate in on-call rotation and support growth initiatives at your center.
Ensure compliance with all licensing and accreditation requirements.
Required Qualifications
Master's Degree in nursing or completion of a PA program with board certification.
Active, unrestricted NP or PA license in the state of practice.
One year of experience in primary care
Experience with electronic medical records, email, and other computer-based programs
Active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and/or Medicaid and Medicare numbers
Use your skills to make an impact
This role is considered patient facing and is part of Humana/Senior Bridge's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
Must be willing to participate in limited on-call coverage by phone

Pay Range
The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
$115,200 - $158,400 per year
This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
