Summary This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. Responsibilities VA offers a comprehensive total rewards package: VA Nurse Total Rewards The Nurse Practitioner (NP) Home-Based Primary Care (HBPC) Nurse Practitioner is accountable for the management and evaluation of a panel of patients under his/her care in the veteran's home setting or via clinical video telehealth. The HBPC NP provides comprehensive preventative and wellness care, as well as the spectrum of acute, episodic, chronic, and end-of-life care to Veterans assigned to his/her HBPC Patient Aligned Care Team (PACT) Team. The NP HBPC uses the nursing process and evidence-based practice to work collaboratively with the Team and the family/caregiver, internal and community-based services, and disciplines involved in providing care to the patient, in developing the patient-driven holistic care plan for life. The NP HBPC provides diagnosis and treatment and provides patient and family health education with a focus on self-management, prevention, and wellness, based on the patient's goals. The NP HBPC is prepared in advanced practice across the health continuum. This role includes participating in and use of research, development, and implementation of health policy, leadership, education, case management, and consultation. The HBPC Nurse Practitioner utilizes preventative clinical guidelines and appropriate standards of care in collaboration with the Home-Based Primary Care Medical Director in providing quality care to patients with complex chronic and acute on chronic health conditions.The NP HBPC: Demonstrates leadership in delivering and improving holistic care through collaborative strategies with other members of the health care team. Utilizes critical judgement in the performance of comprehensive health assessments, differential diagnosis, and the prescribing of pharmacological and non-pharmacological treatments in the direct management of patients with complex acute and complex chronic health conditions. Promotes wellness and prevents illness and injury. In addition, the HPBC Nurse Practitioner serves as a resource to other members of the health care team, providing consultation and education as needed. The HBPC Nurse Practitioner may also function in the dual role of case manager for veteran's assigned to his/her HBPC PACT Team The NP HBPC conducts a comprehensive assessment of patients' medical, physical, and biopsychosocial status including: Obtains a relevant health & medical history Performs a physical examination based on age & history Conducts preventative screening procedures based on age & history Updates evaluation of health status to determine need for further health care Formulates appropriate medical diagnoses based on data obtained Orders appropriate diagnostic procedures and laboratory tests Identifies and prescribes appropriate pharmacological and non-pharmacological agent Provides relevant patient/family education Consults with collaborating health care professionals when doubt exists as to the diagnosis, when expected improvement is not soon apparent, and when specialized therapeutic or diagnostic techniques are indicated. Determines the effectiveness of the plan of care through documentation of outcomes Reassesses and modifies the plan as necessary to achieve medical and health goals Performs any procedures for which she/he has been credentialed The NP HBPC is under the supervision of a Nurse Manager or other immediate supervisor. Clinical supervision is provided by the HBPC Medical Director. Qualifications Advanced Practice Nurse (Nurse Practitioner) Basic Requirements Education: A post-master's certificate or master's or doctoral degree as a Nurse Practitioner. (Applicants pending the completion of educational requirements may be referred and tentatively selected but may not be hired until the education requirements are met.) Must be a graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). NOTE: Foreign Education. Graduates of foreign schools of professional nursing programs, the nurse practitioner degree equivalency must be validated through an Agency recognized degree equivalency evaluation agency, such as the International Consultants of Delaware (ICD). Candidates with equivalent nurse practitioner masters or doctorate level degrees who possess current, full, active, and unrestricted registration and certification will meet the requirement of graduation from an approved school of professional nursing to warrant appointment. License - All APN (NPs) must possess and maintain an active, current, full, and unrestricted APN license to practice as an NP in a State, Territory or Commonwealth of the United States, or in the District of Columbia. - Basic Board Certification - All APN (NPs) must maintain full and current basic board certification in one of the following: Family Nurse Practitioner (FNP), Adult Nurse Practitioner (ANP), Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP), Acute Care Nurse Practitioner (ACNP), Adult-Gerontology Acute Care Nurse Practitioner (AGACNP), or Gerontology Nurse Practitioner (GNP) from one of the following: the American Nurses Credentialing Center (ANCC), American Academy of Nurse Practitioners Certification Board (AANPCB), or other nationally recognized certifying body in the area in which the Nurse Practitioner is academically prepared. Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity.
