Summary The Transitional Patient Advocate will provide a variety of psychosocial assessments and education, advocacy, brief case management services, coordinating linkages with appropriate VA and community service providers, short-term supportive counseling and family services with individual Veterans and families who are experiencing a variety of psychiatric, medical, and social challenges, along with instructing social work students. Responsibilities Total Rewards of a Allied Health Professional Independently evaluates the situations of Veterans and family members/significant others and arrives at a reasoned conclusion. Interprets and explains VA's treatment and benefit programs. Provides comprehensive case management services to OEF/OIF/OND Veterans referred to the program. Determines eligibility for services, completes needs assessment, develops a comprehensive care plan, identifies resources and referrals, and coordinates and monitors services. Coordinates care with other healthcare professionals, including but not limited to physicians, psychiatrists, nurses, chaplains, pharmacists, and other social workers. Identifies those who are at high-risk and in need of acute mental health care and a further mental health evaluation. Assist veterans in coping with acute illness, chronic illness, combat stress, residuals of traumatic brain injury (TBI), community adjustment, addictions, and other health and mental health problems. Works with patients and families who are experiencing a variety of psychiatric, medical and social problems. Independently provides crisis management and develops psychosocial treatment plans in coordination with interdisciplinary team members, including goals for psychosocial clinical treatment. Provides ongoing and supportive counseling to Veterans and, when appropriate, their families, caregivers, or significant others. Independently identifies high-risk patients and provides crisis intervention services. Completes suicide risk screens C-SSRS and completes and documents assessments within the timelines and guidelines of the EPVAHCS Suicide Prevention SOP as required. Provides psychoeducation and interventions regarding a variety of mental health, medical and psychosocial problems. When appropriate and feasible, conducts education and encourages the Veteran/family to advocate on own behalf, thus fostering a sense of independence and empowerment. Participates and assists in planning and executing outreach events for the purpose of providing education and expedited access to enrollment in VA health care. Coordinates referrals/consults for non-institutional services such as HBPC, hospice, transitional and supportive housing, and skilled and non-skilled homecare services in collaboration with the interdisciplinary team members. Provides education on VA entitlements, eligibility, services, and programs, as well as community resources for medical and mental health needs. Educates and refers Veterans - and when appropriate, their families, caregivers, and significant others - to relevant VA or community-based programs and interdisciplinary team members. Manages the evaluation of the professional services provided. Devises methods of obtaining data and evaluates data to determine the effectiveness of treatment. Identifies consistent qualities of successful treatment and of failed treatment. Modifies services to best meet the treatment needs of Veterans and to promote efficient practice. Coordinates Social Work services with other services offered in the treatment program, assuring such services are complementary and comprehensive. Provides case management services to those Veterans at high social or medical risk, serving as the liaison to other treatment programs. (e) Fundamental knowledge of medical and mental health diagnoses, disabilities, and treatment procedures. This includes acute, chronic, and traumatic illnesses/injuries; common medications and their effects/side effects; and medical terminology. Individuals assigned as GS-9 grade level social workers are considered to be at the entry level and are closely supervised, as they are not yet functioning at the independent practice level conferred by independent licensure or certification. Social workers at the GS-9 entry level are typically assigned to program areas that do not require specialized knowledge or experience. GS-11: Experience and Licensure: Appointment to the GS-11 grade level requires completion of a minimum of one year of post-MSW experience equivalent to the GS-9 grade level in the field of health care or other social work-related settings, (VA or non- VA experience) and licensure or certification in a state at the independent practice level, OR, in addition to meeting basic requirements, a doctoral degree in social work from a school of social work may be substituted for the required one year of professional social work experience in a clinical setting. In addition to the experience above, candidates must demonstrate all of the following KSAs: (a) Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services. This is the full performance level. Social workers at this level are licensed or certified to independently practice social work. Incumbents are assigned to all program areas, including but not limited to: inpatient or outpatient medicine, surgery, mental health, neurology, rehabilitation medicine, and geriatrics. Experience is only creditable if it is obtained following graduation with a master's degree in social work and if it includes work as a professional social worker directly related to the position to be filled.
